PSA up after one month on Xtandi

My husband has been taking Zytiga, Lupron, Xgeva and prednisone for two years. The combination worked well with only a few side effects (fatigue and hot flashes), and his PSA became undectable within a few months. When his PSA started going up, his doctor stopped the Zytiga and prednisone and added Xtandi . The Xtandi is making him miserable. He mostly sleeps, has no appetite and lost 7 pounds this month, We just got the results from his last PSA test and his PSA rose again after one month on Xtandi. I'm wondering if we should give up on Xtandi and switch to Keytruda (my hasband has MSI-high), or give Xtandi another month or two.

Has anyone had a similar experience with Xtandi, and if so, what was the outcome?

Keytruda has had near-miraculous results with MSI-hi. Xtandi usually doesn't work for long immediately following Zytiga failure.

Xtandi tried to kill me at full dose. 1/2 dose for about 4 years now... Xtandi after Abi just doesn't seem to work.. some times after a chemo reg/ it will work.. Best of luck..

Thank you for responding. Sounds like Xtandi is not going to work for my husband and he needs to tell the doctor that he wants to change to Keytruda. Out of curiosity though, do you remember how long you were at full dose before going to 1/2 dose and how long it took for your PSA to start trending down?

PSA was trending down after 3 weeks. 25%. 6 months until I was an invalid.. Still on it 4 years later... Best of luck to you. For some it works full strength for years, for others, they just can't stand it. Best of luck to you.

MY experience just the opposite. Lupron failing and PSA jumped from about 2 up to 9.5 in a couple months.Added Xtandi and it drove PSA down below 1. Lasted about 13 mo so dropped and switched to Lynparza, available because I am BRCA2 positive.

Besides some brain fog and exhaustion Xtandi worked well for ME.
