Update on my daddy- 8 years stage four

Hello guys! I wanted to give a little update on my dad.

He was diagnosed stage four prostate cancer in June 2015, with a PSA of 257 and a single lymph node involved. His prostate was so enlarged at the time that he had to have double nephrostomy bags. He started hormone therapy and chemo immediately (6 rounds of taxotere).

He stayed on Lupron and Casodex for five years until it failed and he had 2/3 rounds of provenge (covid interfered 🙄) and started on Xtandi.

He was on Xtandi for three years when his PSA started to rise. It went from .07 to 2.38.

Also in the fall of 21 dad had to have a quadruple bypass and five weeks later was diagnosed with lymphoma in his brain. He had brain surgery, chemo to his brain, covid, several bouts of AFib, and even a heart attack in one year.

Dad is now 82 and the effects of treatment are more evident than ever on him. 😞

Fast forward to the present, when seeing his PSA on the rise his oncologist considered Pluvicto. He ordered a PSMA scan and they discovered that his prostate cancer was limited to a 1.5 cm x 1.5 cm place in his prostate.

So now his oncologist has decided that the next best step is to do targeted radiation.

I'm unsure what kind of radiation the Cleveland clinic uses.. I have a lot of research to do. However, I feel like maybe this is a better option considering my dad's other health concerns?

I'd love to hear what you guys think. On a different note, he just got the results from his latest brain MRI and everything looks great. So I'm very encouraged there!

Thank you! He really is.. it's been one heck of a journey, but he faces everything courageously and with faith! I'm certain he will face this next treatment the same way!

Whole pelvic radiation sounds like a good idea. I think Rahul Tendulkar at Cleveland Clinic or Daniel Spratt at University Hospitals Seidman would be good choices.

Sorry, I don't get it. The oncologist recommended targeted radiation to the relatively small area that is cancerous.

The cancer you can see is definitely not all the cancer there is.

Sounds like SBRT to the affected area is likely. It’s an easy treatment after initial scan usually 3-5 sessions. It’s painless and maybe causes a little fatigue for a week or so afterwards.

I'm not a doctor but I am surprised that he never had a prostatectomy or rad therapy to prostate. So targeted rt to prostate seems like something overdue, considering tumor size. Also the brain lymphoma in brain has me scratching my head because I don't think there are any lymph nodes in the brain. I had lymphoma behind left ear outside skull which I treated with very hot shower to eliminate. He might keep that in mind but if it were to recur I would think it would be inside skull after surgery to brain. Also the way the monster works is devious and needs to be constantly surveiled.

Sorry but I’ve never had Lupron or any ADT. Good luck!

Tell your Dad he looks much younger that 82 years old. He also looks like a guy who has self confidence and of course with the women he always has his pick.
