Current thyroid results from 19th November 2024 Medichecks : T4-15.2 (12-22) T3-4.3 (3.1-6.8) and TSH 0.419 (0.27-4.2) Currently take 5 X100mcgs and 2 X 75mcgs per week of levothyroxine

VITAMIN B12 - Active - 99.5 (>37.5)

VITAMIN D - 50.5 (50-250)

ZINC 8.9 (<10.7)

COPPER 14 (13-23)

IRON 14.5 (10-30)

TIBC 57.4 (45-81)

TRANSFERRIN SATURATION 25.3% (25-45)

FERRITIN 24.30 (30 - 207)

Both hormones are quite low in range, which would give you hypo symptoms. Most people feel well when their hormones are at least 60% through the range, so you definitely have room to increase, especially as you are symptomatic.

Your iron is quite low, both your iron and ferritin (iron storage). You could eat more iron rich foods, but that alone might not be enough to get your levels high enough, so you might consider supplementation. Haem iron is from animal products and is best absorbed (up to 30%) where as non-haem iron found in plant based foods is less well absorbed (3-10%). Having Vitamin C with non-haem food sources will help with iron absorption.

Your folate and Vit B12 are low as well, many people supplement with a good Vitamin B complex, such as Thorne B or Igennus B-complex, which both have bioavailable forms of the B-vitamins for better absorption.

Your Vitamin D is also quite low and just scraping into the range, you should aim for at leat 90nmol/L or above. Remember to also supplement with Vitamin K2 when taking Vitamin D3, as it helps with directing calcium to your bones. If you take the Vit D3 +K2 with magnesium as well, this will help with the absorption, as Mg is a cofactor. You could do a loading dose to get your Vitamin D levels up a bit quicker and then have a maintenance dose to keep it topped up. Many people dose with around 3000IU Vit D3 daily (summer and winter) to keep their Vitamin D at a higher level. Once you are supplementing, you should test your Vitamin D again in a few months to see where you are, and if you need to adjust your dose.

Thank you for your reply. I have been supplementing with iron spray for the last year and eating lots of black pudding but iron/ferritn levels havent budged. I take daily asprin for pain which I think can disrupt iron absorption? Can you advise what optimum level of B12 should be please? I was wondering if it would be worth seeing private doctor to get additional T3 to take alongside levothyroxine as T3 has not increased much since starting levo over a year ago? Thank you

I think you still have room for an increase in levothyroxine and that could bring up your T3 levels along with an increase in T4. I would try this option first and see how your levels change, before thinking about T3.

Optimal levels of B12 are difficult to say, especially when there is no range given for the results. I could find ranges between 38 and 188 pmol/L or 38 - 210 pmol/L. Most people opt for top of the range. Apparently there is no upper tolerable intake limit for Vit B12, as no adverse health effects have been reported from excess intakes (up to 1,000 times the daily recommended allowance).

There is some info about optimal B12 levels, but just be aware that they use a different scale (pg/ml instead of pmol/L; 1 pmol/L = 0.2885 pg/mL.
