New Blood Results - help please

I’ve received my latest blood results and they seem quite random compared to my last set of results. My GP has asked to speak to me and review them but wanted to ask for some advice from the brilliant people here first…

For ref, my levo was increased from 125 to 150mcg in February 24. I’ve been feeling great although tired since I stopped taking ferrous fumarate for my blood test (resumed straight after but still quite whacked).

I was prescribed ferrous fumarate in February 24 and been taking it since, although only half the prescribed dose. Based on my ferritin levels below, I need to double this at least!

I’m hashi’s and vegetarian. I stopped Basic B complex and FF ahead of my test, which was taken at 8am - the same as I usually do. I did take a folate supplement (first time doing so) a couple of days ahead of the test, as advised on here to help combat fatigue caused by pausing Basic B and FF ahead of test).

Thyroid function test

Serum TSH level 0.01 mu/L [0.3 - 5.0]; Below low reference limit

Serum free T4 level 16.5 pmol/L [7.9 - 16.0]; Above high reference limit

Serum free triiodothyronine level 5.6 pmol/L [3.8 - 6.0]

Serum vitamin B12 level 570 ng/L [120.0 - 900.0]; Adequate B12 stores.If the patient is on IM B12 therapy, it is not necessary to monitor B12 levels

GP has said this B12 result is abnormal?

Serum ferritin level 47 ng/mL [15.0 - 300.0]; Ferritin >15 and <100ng/mL. With low Hb and MCV,

Serum folate level > 20 ug/L [2.5 - 9999.0]

If you allowed 24hrs between last dose of levo and blood test then its not ideal to have FT4 above range.

FT4 is only fractionally raised so you could just drop 25mcgs on 2 days a week.

FT3 looks good at 82% of range, possibly also a tiny bit too much.

Has vit D been tested lately and are you supplementing?

B12 result looks good, not sure why GP has said abnormal. It would be good to see an active B12 test at some point which is a better measure than serum B12. Might have to do that privately.

Was your TSH suppressed on previous dose? GP may not be happy about that.

You could try taking iron with orange juice or vit C to help it absorb better but ferritin can be very slow to raise.

Folate is good.

My last TSH result in Feb was 0.46 and my GP increased my levo from 125 to 150 as my iron was so low at the time. But I think your idea to take 125mcg 2x a week is great, so I’ll do this and check GP is happy too. I’ll keep going with the FF and increase my dose with orange juice

I do supplement Vit D but not sure why it wasn’t tested - it was last time and levels were good.
