Hello, I have some new results and would be grateful for any thoughts!

Iron 10.9 umol/L (Range: 10 - 30)

TIBC 56.5 umol/L (Range: 45 - 81)

UIBC 45.6 umol/L (Range: 13 - 56)

Transferrin Saturation X 19.3 % (Range: 25 - 45)

Ferritin X 14.90 ug/L (Range: 30 - 207)

Vitamins

Folate - Serum 11.8 nmol/L (Range: > 7)

Vitamin B12 - Active 68.9 pmol/L

Please note change of reference range 23 Oct 2022

(Range: 37.5 - 188)

Vitamin D X 43.1 nmol/L

Total 25(OH) vitamin D < 25 nmol/L is deficient.

Total 25(OH)D of 25-50 nmol/L may be inadequate in some people.

Total 25(OH)D > 50 nmol/L is sufficient in most individuals.

is it safe to say you still feel symptomatic?

Per past posts, I think you are on 50 mcgs Levo for the past 4 weeks? You are still under-replaced, which is to be expected as you are on a starter dose, so also assume you are planning to titrate towards 75 mcgs in another couple weeks?

Your iron panel is severely deficient - both iron and ferritin, and your transferrin saturation is too low which will be impacting your body’s ability to use T3.

D also needs a supplement plan to get you above 100.

Feel awful most of the time - heart palpitations, shortness of breath, freezing cold, fatigue.... Yes, been taking 50mcgs since 26th January I have appointment with endocrinologist on 19th but since Saturday 24th I have been adding 12.5mcgs every other day and will do every day in a few more days.

Since both your iron and ferritin are low, you can supplement iron, and should right away.

I had the same situation and found this graphic which explained a lot!

Yes have all of those! I have been taking iron drops regularly since last october but obviously hasnt worked!

How much iron and how frequently?

about 3 times a week of 3 drops of a 22 mg but just saw that that is only equal to 0.4ml so nowhere near the 22mgs each time - would need to have 1.2ml to get 22mgs - thats what ive been doing wrong!

How often can you check your iron panel?

Are you taking iron 4 hours away from your Levo?

Are you vegetarian?

i get it done privately so can get it tested regularly. Only eat chicken and some fish - no red meat. Yes, take iron at night and levo in the morning

Let me know if you are open to taking a heme iron supplement instead (basically dehydrated beef spleen… but not as gross as it sounds, it’s just powdery in a clear capsule), check out the Three Arrows.

It absorbs at least 3x better than non-heme iron sources, does not cause any stomach upset, and can be taken with our without food.

I started my daughter on it, and because I knew it was so well absorbed, I made sure to recheck levels at 5-6 weeks. Lo and behold, her iron went to top of range in that short time.

Now….. in a month or so when you do get your iron up, feel free to post again and tag me. Because it will take months/years to increase your ferritin. And that’s a trickier balancing act.

It is a great product, so many people on the board use it, me and my daughter included. Do not be put off by the website - which feels a little unpolished!

For reference, my daughter is 140 pounds and took 2 capsules a day (2 x 40 mgs) for that 5-6 weeks.

Lastly, very important. Excess iron is toxic and causes irreversible organ damage. Our bodies have no way to excrete excess iron, so it settles in our organs.

You want to target about 55 - 70% through range. As you get close, you will need to stop supplementing and focus on an iron rich diet. Your ferritin will not increase quickly at all. Those parts are a whole other endeavor!

I am not a B/folate expert but I think those are dire as well.

Which all makes sense as your hypo is interfering with absorption of all of them.

Others on this board have more developed responses that run through the how much & when’s of how you can tackle it all!

I was very scattered replying to the back and forth. Wanted to sum it up. Somewhat because you still need advice on B and Folate which I don’t know enough about to give you.

Levo - titrating to 62.5 is a good plan. Don’t be tempted to rush into 75 even though you will get there soon. Thyroid hormones in our body take 6-8 weeks to settle.

Iron - tackle with three arrows when it comes, continue your drops if you’d like, but less than 20 mgs a day will likely do nothing for you, but as I said above, stay vigilant. With non heme supplements I’ve heard testing every three months is a good target, but no longer.

D - use that calculator above! Target 100. Search the forum for good advice on D (ie, there’s a mouth spray recommended) and ALSO…

Vitamin K - Take your D WITH vitamin K (mk-7 to be specific), which helps the D get to your bones and teeth where you need it. Otherwise it could calcify in your blood/veins which doesn’t do you any good at all! Recommended on K is at least 100 mcgs a day but I’ve also read it is 100 mcgs per 1000 ius of D (personally to correct my current D deficiency I’m taking 7500 d with 400 mk-7 just to hit somewhere in the middle, but that’s just me)

Also - I know nothing about these, but I think you will need to supplement these:

You are so early days that you have a lot of very normal and expected issues, and the above actions are solid … then after a few time periods of adding/increasing, come back and the board can help you fine tune.

So if not currently taking any vitamin D….start with that ….then add magnesium

Followed by B12 and folate

By then your 3 arrows iron supplement should have arrived

GP should prescribe 1600iu everyday for 6 months, but likely to improve faster if self supplement at higher dose….perhaps 2000iu - 3000iu daily

Some areas will prescribe to bring levels to 75nmol or even 80nmol

A week later add a separate vitamin B Complex

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)
