Full set of test results finally

After some delays and other issues, finally got test done and more delays before getting back here.

Test Result Normal Range

Biochemistry

CRP 1.19 <5.0 mg/L

Ferritin 233.0 30 - 400 ug/L

Magnesium 0.97 0.66 - 0.99 mmol/L

Hormones

Cortisol (Random) 492.0 6am - 10am 166 - 507 nmol/L

4pm - 8pm 73.8 - 291

Thyroid Function

TSH 1.29 0.27 - 4.20 mU/L

T4 Total 81.4 66 - 181 nmol/L

Free T4 14.5 12.0 - 22.0 pmol/L

Free T3 4.21 3.1 - 6.8 pmol/L

Immunology

Anti-Thyroidperoxidase abs H 232 <34 IU/mL

Anti-Thyroglobulin Abs 35 <115 IU/mL

Vitamins

Vitamin D (25 OH) 68 Optimal 75-200 nmol/L

Adequate 50-<75

Insufficient 25 -<50

Deficient <25

B12-Active 61.0 37.5 - 150 pmol/L

Serum Folate 12.60 8.83 - 60.8 nmol/L

formatting has gone astray, but think it s readable.

Actually it is the lowest TSH I have ever recorded which does not help my argument.

The Anti-thyroidperoxidase reading I gather confirms Hashimoto’s .

Everything broadly OK I think. I have gone back in vitamin D - will now take year round rather than just winter months.

Comments appreciated. I am trying to persuade GP to up my does from 75 to 100 as I never had any symptoms that I was aware of ( blood test was only diagnosis) and saw no change on taking levo.

Cholesterol climbing steadily over time taking levo. Suspect need to get T3 near top of range.

Thanks

What time of day was this blood test taken & did you remember to leave 24 hours since your last Levo dose? Highest TSH is at around 9am or earlier so we recommend testing as close as possible to that time.

Free T4 (fT4) 14.5 pmol/L (12 - 22) 25.0%

Free T3 (fT3) 4.21 pmol/L (3.1 - 6.8) 30.0%

Your free hormones are pretty low. On Levo alone I'd expect to see your FT4 in the upper part of the range at 80%+.

Your TSH being over 1 still has a little room for an increase.

Low thyroid hormone will raise your cholesterol so do drive that home to your GP. You have scope to increase your thyroid levels and therefore decrease your cholesterol.

Your B12 & folate are both on the low side. Working on B12 in particular may help raise your TSH a little.

I would suggest a B complex supplement of the methyl/active type which would help both your B12 & folate levels and helps keep the B's in balance. This B complex has all the right vitamins at a not unreasonable cost for 90 days supply. amazon.co.uk/Liposomal-Soft...

Many with autoimmune thyroid disease aka Hashimoto's benefit from a gluten free diet. A smaller percentage of those also need to remove dairy from their diet to feel well. These are intolerances and will not show up on any blood test.

Recommended blood test protocol: Test at 9am (or as close as possible), fasting, last levo dose 24hrs before the blood draw & no biotin containing supplements for 3-7 days (Biotin can interfere with thyroid blood results as it is used in the testing process)? Testing like this gives consistency in your results and will show stable blood levels of hormone and highest TSH which varies throughout the day. Taking Levo just prior to blood draw can show a falsely elevated result and your GP/Endo might change your dose incorrectly as a result.

Taken at 7am only available time for appointment over whole month 24 hours after Levo. Will read and digest observations later but many thanks for the response - off for MRI scan now 7pm on a Sunday on NHS…….

B12-Active 61.0 37.5 - 150 pmol/L

Serum Folate 12.60 8.83 - 60.8 nmol/L

both very low

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and a week later add a separate vitamin B Complex

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Other options

healthunlocked.com/thyroidu....

Igennus B complex popular option too. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

endocrinenews.endocrine.org...

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Are you taking any vitamin D?

Jasper79 in reply toSlowDragon

Thanks - will study in more detail tomorrow . Yes taking vitamin D 10mcg at moment. Previously only in winter but, seeing blood test, all year now.

10mcg is only 400iu

Likely to need at least 1000-2000iu daily

Probably higher dose in winter than summer

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

mm.wirral.nhs.uk/document_u...

But with Hashimoto’s, improving to around 80nmol or 100nmol by self supplementing may be better

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Re vt D, yes going to up to 1000 IU shortly. Taking what I had in cupboard at moment.

I take 300mg Magnesium Citrate other reasons.

Will read your comments in more detail at leisure.

Thanks again

If it’s tablets or gels make sure you take them at least 4 hours away from levothyroxine

have ordered Methyl B Complex from Bio Care who I use for another product. Its seems to have everything recommended.

Thanks again for the advice. Trying to keep costs down, so hoping this alone will make a difference.

SlowDragon profile image
SlowDragonAdministrator in reply toJasper79

You need to start on separate B12 first and continue with B12 and vitamin B complex until Active B12 is over 70 or serum B12 over 500

Jasper79 profile image
Jasper79 in reply toSlowDragon

Have now ordered separate B12 so will take this for a week and then add B Complex back in. Plan new full set of tests about 4 weeks after starting 100μg levo so will know B12 etc then.

Limit to how many tests are practical especially as I have no success with finger prick collection.

Thanks
