B12 has improved really well. No need for separate B12 supplement now, I would just continue with the B Complex which should maintain your level.

Vit D is improving. Is your D3 and K2 a combined supplement or two separate supplements?
If separate supplements are they both oil based, eg softgel with olive oil, etc? If not they should be taken at different times of the day and each one with some fat for absorption.
The Vit D Council, Vit D Society and Grassroots Health recommend aiming for 100-150nmol/L although at least 125nmol/L is recommended by Grassroots Health.

Results suggest poor conversion of T4 to T3 as FT4 is 103% of range and FT3 is only 40.54% through range. As nutrients are at pretty good levels then maybe consider addition of some T3 to a slightly reduced dose of Levo.
The vit D K2 is Nature's Best combined, 2 capsules of 1000iu.
You might want to consider 3 x 1,000iu during the winter months as we can't make any D3 naturally at the moment, see if that gets you up to the recommended level.
Always have the same brand Levo , which is 150 accord and 25 Northstar teva.
For next prescription I have changed to chemists for u as they have told me they can get Mercury Pharma. So we can only try to see if the change happens.
My skin is so terribly itchy.
Trying really hard with not eating gluten, that's not to bad, but dairy is difficult, I love eggs. We have been good with our diet, we've been following the harcomb diet and each receipie outlines which ingredients contain dairy etc, not lost any weight mind🤣.

Struggling with aches and pains in feet and ankles and have a very swollen knee as I have inflammation around it, with a meniscus tear and cartilage rubbing together.
My bloods never show any raised inflammatory markers and I don't have the Rhumatoid gene.
I have tried various inflammatory medication over the last couple of years, but due to side effects they haven't worked, and am due to start a Jak inhibitor next week. Rhumatology has told me last try and then she will diagnose with fibromyalgia.
I am due for knee aspiration and cortisone at some point, hopefully soon.
