I understand that you have taken time to complete this and am very sorry about this omission which I must reassure you is very rare, especially with a thyroid test where the background information is so important.
I understand you had RAI treatment for overactive thyroid and are now on levothyroxine for treatment of underactive thyroid as a result.
Your thyroid stimulating hormone is in the upper half of the normal range and you have a low free T3.
People with hypothyroidism can feel better when their thyroid stimulating hormone is maintained in the lower half of the normal range and given your free T3 is low also it may be worth discussing this with your doctor.
If you feel well then you can look to retest in 3 months to see if things have come back to normal. If you feel you have some thyroid symptoms, however, then you should discuss this further with your doctor who can advise you as to whether you need an increase in your levothyroxine dose.
The rise in thyroid peroxidase antibodies is in keeping with autoimmune thyroid disease. This can be seen with Hashimoto's (underactive thyroid) but also in Graves which leads to an overactive thyroid.
You have a healthy level of iron, folate, B12, folate and vitamin D. Your CRP level is normal, suggesting no significant inflammation within the body.
Although not tested here you may wish to consider checking your Zinc or Selenium levels or taking an over counter supplement for these as these are needed for thyroxine to T3
