Hi there,

Please can someone help I have just had my blood results and GP is concerned my TSH is too low. I am shaky, have migraines, extreme fatigue , dizziness and vertigo. I take 100 my Levothyroxine on Tue- Thursday and 75mg on A Monday and Thursday , plus 5mg Liothyronine.

Blood results as follows:

TSH less than 0.02 Range 0.27-4.2

T4: 18.8 Range 12-22

T3: 4.7 Range 3.1-6.8

Ferritin : 54 Range 13-150

Folate: 7.4 Range 3.9-26.8

B12 : 552 Range 197-771

Do you have a free T3 and Free T4? The plain T3 and 4 don’t represent what you are really working with as they include protein bound hormone.

Your labs on iron are missing a saturation %, and a TIBC and total iron. You really can’t diagnose iron issues by just ferritin. Factors that influence your ferritin levels, such as MTHFR status and body inflammation, are independent of your true iron status. Ferritin can look high when you’re actually low on iron due to inflammation. Conversely, Ferritin can appear low when you’re iron status is good due to MTHFR snps. Try to get those other labs for a full picture of you your iron status! Same with a free T3 and free T4. I’d say get a reverse T3 because it tells you quite a lot about your thyroid health, but understand it isn’t done in the UK… correct?

I am so sorry I’m only seeing your reply now much appreciated.

I honestly don’t know if it was reverse t3 or normal as it was Gp who did it as I have only had telephone referrals with consultant since pandemic.

I am just shattered maybe I need to take 10 mg t3 as 100 was too much T4 in the summer.

Chances are good that the 100 mcg of T4 with no T3 created a high reverse T3, that can make you feel terrible! I had that when I was only on Armour thyroid 240 mg. Had to drop it to 60 mg and slowly add generic T3 to it to get to feeling better. My cause was kinda low iron, so I take Floradix and slowLy (over 3 months), get my T3 to 85 mcg daily. I spread it out during the day. Take thyroid meds at 8am/12:30pm/ and 5:30 pm.

It has made all the difference in the world in how I feel! Your vitamin B-12 levels look too low also. Good luck to you.

Oh forgot to add, please don’t be afraid of the T3, but take it every 4 hours x3 per day, and get your iron and b-12 good. No more than 60 mg T4 if you have high reverse T3. Learned the hard way…😉

Hope you are feeling better now that’s awful. No I was taking 20 mg t3 and 100 T4 then I reduced it to t3 10 then 5mg and from Sept it has been 75 mg T4 on a Monday and Thursday and 100mg rest of the time.

I think my b12 is low too but GP says it’s grand they just don’t understand as I have had vertigo last two years and seeing chiro to help. I take vitamin d spray as I haven’t had it checked in two years.
