Hi I have had my recent bloods back, after 6 years of feeling dizzy (like being on a boat) and difficulty thinking at times with anxiety I would be grateful if anybody can help. My blood sugar also dips which makes my symptoms worse. The doctor said these were all fine.

B12 is very low

What vitamin supplements are you currently taking

Have you had vitamin D tested

Or coeliac blood test

As thyroid antibodies are high this confirms autoimmune thyroid disease also called Hashimoto’s

Gluten intolerance very common

Get coeliac blood test done before considering trial on strictly gluten free diet

Was thyroid testing done as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

Do you always get same brand levothyroxine at each prescription

Hi, I'm not vegetarian or vegan. I don't take any vitamin supplement. I don't think I have have vitamin D tested. I was tested for coeliac and it was negative. The test was done at 10 am and I had taken my meds at 7 am as usual. I had also had my breakfast. I take mercury pharma 25's to make up my 175 daily dose. I have had trouble with other brands and can only get Merc in 25's so having to take 7 a day.

Mercury Pharma make 25mcg, 50mcg and 100mcg tablets

Last dose levothyroxine should be 24 hours before test

So Ft4 is falsely high

Many/most thyroid patients find they need to supplement to maintain OPTIMAL vitamin levels

Only 5% of autoimmune thyroid patients are coeliac

A further 80% find strictly gluten free diet helps or is essential……so it’s always worth trying

Request GP test for Pernicious Anaemia before starting any vitamin B12 and daily vitamin B complex

Come back with new post once you get vitamin D test result

Hi thanks for your help, I have a right job getting even the 25's in mercury, I have to travel 20 miles to a different chemist to get my prescription every 4 weeks. Another person on here has said my cortisol is high. I have lived with this for 6 years now but recently I'm finding my memory and ability to think is terrible so I need to get to the bottom of it.

Yes…finding a helpful pharmacy can be difficult

I drive 10 miles to small independent pharmacy to always get Mercury Pharma - 100mcg and 25mcg tablets

Your GP could/should do prescription to cover 2 months at a time

It’s essential we have OPTIMAL vitamin levels

Frequently necessary to supplement continuously to maintain good levels

Many people find strictly gluten free diet can (slowly) improve brain fog……can take several months to see slow steady improvement

Personally I found brain fog improved after about a year gluten free

Gluten crosses blood/brain barrier

Brain fog is a good word for it, I do have the odd day or few hours that I'm free of it but that makes me realize how bad it is living with it haha. The fog gets worse when I don't sleep or if I have to take Teva. I will try going gluten free.

Many people find Levothyroxine brands are not interchangeable.

Many patients do NOT get on well with Teva brand of Levothyroxine.

Teva contains mannitol as a filler, which seems to be possible cause of problems. Teva is the only brand that makes 75mcg tablet. So if avoiding Teva for 75mcg dose ask for 25mcg to add to 50mcg or just extra 50mcg tablets to cut in half

Most easily available (and often most easily tolerated) are Mercury Pharma or Accord

Mercury Pharma make 25mcg, 50mcg and 100mcg tablets

Accord only make 50mcg and 100mcg tablets

Accord is also boxed as Almus via Boots, and Northstar 50mcg and 100mcg via Lloyds ....but Accord doesn’t make 25mcg tablets

beware 25mcg Northstar is Teva

Once you find a brand that suits you, best to make sure to only get that one at each prescription.

Watch out for brand change when dose is increased or at repeat prescription.

If a patient reports persistent symptoms when switching between different levothyroxine tablet formulations, consider consistently prescribing a specific product known to be well tolerated by the patient. If symptoms or poor control of thyroid function persist (despite adhering to a specific product), consider prescribing levothyroxine in an oral solution formulation.

Physicians should: 1) alert patients that preparations may be switched at the pharmacy; 2) encourage patients to ask to remain on the same preparation at every pharmacy refill; and 3) make sure patients understand the need to have their TSH retested and the potential for dosing readjusted every time their LT4 preparation is switched (18).

I have just looked through my notes and I have been tested for anemia, is pernicious anemia different? My test results said blood count normal.

There are many types of anemia.

The most common is probably iron deficiency anemia.

Pernicious anemia is entirely different and is due to insufficient vitamin B12 being absorbed - because the stomach is not making sufficient of a substance called Intrinsic Factor to help B12 get carried from stomach to terminal ileum where it can be absorbed.

There is no simple, single test for Pernicious Anemia.

It is probably right to have an Intrinsic Factor antibodies test. Trouble is, around 50% of people with PA get a negative result from this test. But it is likely the right next step.

Has GP said anything about it being so high? It suggests further investigation. I'll ask Hidden to comment if she's around, she's very good with blood cortisol results.

Hi, the doctor said it's fine, I have since gone private and been told my results are fine. I'm really fed up with it, my brain just doesn't seem to work at times.

Over range is suspicious. If this was a test before 10am then it's not fine.
