Your FT4 and FT3 are too low, I would say you need an increase on your Levo. If you get your FT4 up to the top third of the range (19ish) and your FT3 remains very low then you don't convert well enough and could consider adding some T3.

Your Tg antibodies are raised confirming autoimmune thyroid disease aka Hashimoto's. You should adopt a strict gluten free diet and supplement with selenium l-selenomethionine 200mcg daily to help reduce antibodies.

Active B12 is OK.

Folate is too low, it should be at least half way through the range. A good B Complex such as Thorne Basic B will help raise your level.

Ferritin should be at least 70, preferably half way through the range. Eating liver regularly will help raise and maintain a good level.

Vit D is too low, the Vit D Council recommends a level of 100-150nmol/L. You can supplement with Better You oral spray which is better absorbed when you have Hashi's. Start with 6000iu daily for a couple of months then reduce to 3000iu and retest 3 months after starting. When you've reached the recommended level you need to find your maintenance dose by trial and error, it may be 2000iu or more or less. Retest twice a year to keep within the recommended range.

Also take D3's important cofactors K2-mk7 and magnesium. Take D3 and magnesium 4 hours away from thyroid meds and take D3 and k2 with the fattiest meal of the day.

I think those brands will only be available online.

Thorne Basic B seems to be cheapest on Amazon at the moment. You'll have to Google for Betteryou spray and compare prices as quite a few places sell it.

For K2-mk7 I like Healthy Origins brand so Google and compare prices for that.

Magnesium you have to decide which firm is best for you

and look for a good brand. I use magnesium citrate as I need the laxative effect and buy Natural Calm Original powder from Amazon when the price is reasonable.

Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten

According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly.

Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)

But don't be surprised that GP or endo never mention gut, gluten or low vitamins. Hashimoto's gut and gluten connection is very poorly understood

Changing to a strictly gluten free diet may help reduce symptoms. Certainly worth trying for 3-6 months, if it helps stick on it
