Now, I'm not a doctor but this really doesn't look too stellar, does it? I remember somebody mentioning that hypothyroidism could affect the B12 reading and that the active B12 reading could be much lower. Have I got that right, and is there something that I could print out to show my GP when I ask for an active B12 test?
Thyroid panels all in the upper normal range now.

Hose, you're right that you may only be transporting 20% of the B12 but I doubt your GP will refer you to St. Thomas' hospital for the active B12 test as you are comfortably within range. My B12 was similar to yours and retested over 700 when supplementing 1,200mcg methylcobalamin daily for 8 weeks. Don't supplement if you intend to retest. Increased folate may be seen with pernicious anaemia, vegetarian diets, or with a condition called bacterial overgrowth syndrome where bacteria multiply in the upper bowels. labtestsonline.org.uk/under...
You should be prescribed high dose vitD3 for your vitamin deficiency and iron for your ferritin deficiency. High dose vitamin C of 500-1,000mg should be taken with each iron tablet to aid absorption.
