Serum vitamin D level (XE2e7) 37 nmol/L [25 - 300]

Serum folate level (42U5.) 1.5 ng/ml [4 - 27]

Serum vitamin B12 level (XE2pf) 341 ng/L [130 - 800]

Serum ferritin level (XE24r) 125 ng/ml [30 - 400]

Looking at previous posts you’re on levothyroxine

Recently had dose increase in levothyroxine to 100mcg

Likely to need further increases over coming months

Meanwhile essential to improve low vitamin levels

Were these tests done by GP?

Folic acid is deficient

B12 too low

Vitamin D insufficiency

Ferritin on low side for a young man

Low vitamins are direct result of being hypothyroid. And need improving to optimal levels

GP should prescribe 1600iu everyday for 6 months

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

But with thyroid disease , improving to around 80nmol or 100nmol by self supplementing may be better

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function.

There’s a version made that also contains vitamin K2 Mk7.

It’s trial and error what dose we need, with thyroid issues we frequently need higher dose than average

Suggest you start with 3000iu per day

Retest in 3-4 months

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement

Week after starting B12 add folic acid prescribed by GP

once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with folic acid from GP

Once you finish folic acid prescription, then start a good quality daily B Complex indefinitely

Important B12 is good level before adding folate/folic acid

It is vital if you intend to supplement both B12 and folate that B12 is started a week before the folate.

Suggest you get B12 supplement and start this few days before you start GP prescription for folic acid

But also….Look at increasing leafy green vegetables in your diet

Once you finish prescription folic acid

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) 

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) 

Thorne currently difficult to find at reasonable price, should be around £20 

If you want to try a different brand in the meantime, one with virtually identical doses of the ingredients, and bioavailable too, then take a look at Vitablossom Liposomal B Complex. Amazon sometimes has it branded Vitablossom but it's also available there branded as Yipmai, it's the same supplement

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate methyl folate supplement and continue separate B12

only add one supplement at a time

Then wait 10-14 days to assess before adding another

Suggest you start with vitamin D, then B12, then folic acid

Meanwhile also increase iron rich foods in your diet, to improve ferritin levels

In a previous post you said you had been prescribed 5mg Folic Acid for folate deficiency. Your blood result has actually dropped. Have you been taking the prescribed folic acid?

I got told to stop as my results were adequate at 4

I'm sorry but 4 is in no way adequate for a folate level regardless of the range. GP's are simply hopeless when it comes to vitamins. You can either show this result to your GP and get represcribed Folic Acid or take charge of this yourself ) which wouldn't be a bad idea!) and go buy a good B complex. You might need to top that up with some extra methylfolate over time. See SlowDragons reply with advice.

The moment you stop the supplement you will start dropping levels again. You may well need to supplement this for life. Something GP's do not understand.

Prioritise getting your vitamin levels right and you will benefit with better health.

yeah I agree, I feel some of my symptoms that I still get might be due to these deficiencies but it just seems the doctor don’t care.

Ignore your doctor on this, trust me. You need to get those levels up to optimal.
