Another Pluvicto failure
My dad was fortunate enough to get a go, but after two infusions his
PSA
did a gigantic jump from 42 to 119, over only two weeks. His doctors are puzzled, and so are we.

Newly Diagnosed high risk PCa at age 52
Here's my test history: 2017 - first high
PSA
test of 2.7. DRE normal 2017 - 2019 gradually rising
PSA
up to 3.2. Annual DREs still normal 2021 -
PSA
up to 4.7, DRE normal. Opted for MRI instead of biopsy. 3T MRI found extensive evidence of prostatitis, but no indications of neoplasia.

PSA
last month stood at 0.008. 8.5 years and counting, guess I'm lucky. Approximately one year after DX I decided I needed something to keep me going so after much nudging the Manhattan Community College accepted me.

PSA
426/Gleason 8. Had been on Abiraterone Acetate (AA) along with Lupron and Prednisone. My
PSA
dropped to 0.1 until recently I became castrate resistant. I was advised by my team at MD Anderson that my cancer was a strong variant type.

PSA post salvage radiation
He had clean margins and reached undetectable
PSA
after Prostatectomy. Near the end of 5th year after surgery his
PSA
became detectable. I’m confused, If clean margins how would cancer be outside the prostate? He did salvage radiation at .09
PSA
.

Gleason score 8 and lots of questions...
PSA
7 after a reading of 4 just four months prior. My biopsy results graded out at a Gleason of 8 (4+4) on three of the twelve cores taken and 7 on one other (out of twelve). So a Gleason of 8 overall.

Chemo with Docetaxel - side effects
PSA
peaked 3 months after RP at 156. Was all the way down to 2.9 day of second Docetaxel infusion. 64 years old and good health except for the MPC. My question is about the Docetaxel side effects. After infusion #1, I took nausea medicine for a couple days as a precaution but never felt sick.

Question
I also don't know if this is in response to her seeing the blood test results earlier than I did...which indicated, sadly, that his
PSA
jumped from 1.09 to 6.92. Are xrays ever done for those who can't tolerate scans?

just diagnosed with CKD stage 3.
Due to very slight rise in
PSA
they did a Pet scan which showed kidney cancer, what a shock no symptoms at all, apparently until late stages there is no symptoms. Anyway he has had Nephrectomy and awaiting scan and blood results next week to ensure no cancer cells are anywhere else in body .

About Radiation After Radical Prostatectomy
Do they use radiation as a tool to bring
psa
down in metastatic men? and or radiation is only used as palliative for bones and pain?

Advice for next steps for 82 year old dad
Upon diagnosis his
PSA
was 257 and he had a single lymph node found to be involved. He had chemo and started on lupron + bicalumatide. In 2020 the medicine began to fail and he had provenge and started on Xtandi. Now his Xtandi is failing.

Xtandi is failing
I had a 2nd scare in April/May 2022 when my
PSA
rose to 39 and was doubling every 3 months. I started Xtandi in about May 2022 and got to 0.08 in a few months but
PSA
is increasing at a rate of around 10 times every 3 months and is now 8.7; that is concerning.

PSA
number. (Easy for him to say). I was diagnosed Nov 2004 at 67 yrs old. I HAVE NEVER HAD ANY SYMPTOMS.

Treatment Decision: Now considering surgery
As a reminder I am Gleason 8/9 (all 12 cores) with a
PSA
that has been hovering around 25 since October. I am paraphrasing Dr. Brachy but he essentially said, "I rarely recommend surgery but it does happen.

PSA up today
Had a blood test today and everything looks good except
PSA
went from 0.01 to 0.03, bilirubin at 20 and ALT at 5.0. I go for my next lupron should next Tuesday. Is there anything I should be concerned about and what questions should I ask?
