My Scan Story
Hi all...great info shared here that has helped me after my RP...I went from
PSA
of 2.4 in 2021 to a 4.91 in 10/23..Dr did a 12 core biopsy cause of 3 bro's and Dad all with PC. 2 cores came back with Gleason 7 and an 8. They did a PSMA test with nuclear dye that showed it confined to gland.

EBRT - Is it the right thing and what to expect?
After DX in November 2018 with Stage IV and Gleason of 5+4 and 4+5, and 5 years of ADT of Lupron (then Orgovyx), prednisone 5mg, and Zytiga 250mg with low fat breakfast, my
PSA
stayed stable at 0.4 or 0.5. At end of 2013,
PSA
started rising to latest value of 0.84.

Low PSA + high Gleason-FYI
For instance, I had an initial
PSA
of 4.8 (4.6 after retesting), and I’m Gleason 9. Three of my doctors have mentioned it to me in passing (most recently today), which led me to Google it.

Becoming mCRPC despite falling PSA
He was diagnosed in summer 2022 with stage 4, Gleason 4+4,
PSA
of 122 and many osteoblastic bone metastases throughout the whole skeleton (very high volume) He did the triplet therapy, which lowered his
PSA
to 0.4 (January 2023).

PSA Roller Coaster ride!
I ask them if they would do a
PSA
draw. they did. It came back @ <0.01. What do we do, one lab said 2.22 the other undetectable. The Dr said we should still move forward with a PSMA-Pet Scan. So I have it scheduled. Then check the
PSA
in a month.

PSA testing - time taken to separate serum
The reason being that over time
PSA
levels will reduce if the sample remains whole ie unspun.

PSA
was 19 at time of diagnosis, Gleason 10. BRCA negative.
PSA
is now at 0.2 since starting hormone therapy. Feeling good, no pain. Still works (owns his own company). Started hormone therapy (30 days of pills, one transfusion so far - next in 4 months). He’s feeling fine.

PSA on the rise
(I had taxetere in 2021) I get my
PSA
checked every three months right before lupron injection, so I did the routine blood work. I was suspisious that my
PSA
was probably on the rise as I have had some back pain in the last 4 weeks or so.

My husband is BRCA2 positive, had a prostatectomy, salvage radiation and now his
PSA
is starting to creep back up. I know it’s too early to take additional treatment steps but we want to get set up with a care center that provides the best/latest treatment options, guidance etc.

Has xtandi failed?
I had my lupron injection Tuesday and
PSA
checked.
PSA
0.09, three months ago
PSA
was 0.06. I asked my doctor if xtandi has failed, he told me not necessarily. He told me once your
PSA
reached its lowest point it fluctuates up and down until it stabilized.

Kishan's 6 month Androgen Annihilation Therapy Study for metastatic PCa
"The results confirm that addition of metastasis-directed SBRT to highly potent systemic therapy can maintain low
PSA
after testosterone recovery, although further studies are needed to clarify the optimal systemic therapy regimen."

How Long To Discontinue 5-ARIs and/or Select Supplements to "Unmask" PSA?
Purportedly some substances may "mask"
PSA
. I believe the general rule of thumb is on 5-ARIs like Avodart (Dutasteride) you are supposed to double the
PSA
reading to get the "real"
PSA
?
