Update recent PSMA
So expect to add to ADT, abi or enza or darolutamide in a short while, with
PSA
going up on a fast double. Trying to find out if Nubeqa is worth it regarding technical difference in progression and overall survival, compared to abiraterone. Abi may be better in some ways. thanks.

Provenge (only) - impact on overall survival
Latest
PSA
was 0.918, but
PSA
is not good indicator for me, highest it got during cancer was 1.1 Any thoughts from Allen or the group? (My profile history needed updating so it's blank right now while I edit it down to make details easy to read.)

What do you think?
As my
PSA
rose I found my best treatment was Xtandi. I had pretty good results from 2017 to 2020 , then my
PSA
moved upward again. Looks like I’m going to fail with Nubeqa, which I have been on for 2 1/2 months, my
PSA
jumped from 176 to 194 and then up to 224.

SRBT and EXTEND trial
They only evaluated "progression-free survival" which, at 22 months, was almost entirely lack of
PSA
progression. They claimed that the lack of
PSA
progression made it safe to give patients a break from ADT".

FG-3246 shows promising results in phase 1 trial
As mono therapy "data from the study showed that 36% of evaluable patients experienced at least a 50% reduction in prostate-specific antigen (
PSA
) levels from baseline. Among RECIST-evaluable patients, 20% demonstrated a partial response or a reduction in tumor size of at least 30%.

PSA increase nearly one year after triplet therapy (mHSPC), what's next?
(Pro177Arg) in exon 5 of the gene TP53( NM_000546.5)" if that means anything to you all I've researched a bit but I don't know how to take a
PSA
increase like his considering his initial
PSA
diagnosis was so high. It was also a very weird shift in
PSA
over the months.

Is a Liquid biopsy of ctDNA with PSA = 6 and 4 bone mets, useful?
Hello friends I have to pay upfront the costs but I don't know if nothing will be found in a liquid biopsy with
PSA
of 6.5 What is your experience with liquid biopsy? And did anyone have a bone metastatic solid biopsy ? Thank you 🤗🙌

bicalutamide no longer as effective?
After more than two years of bicalutamide monotherapy and repeated
PSA
<0.1 I just received a 0.6. Before the bicalutamide my PSMA scan showed activity in only the seminal vesicle region.
PSA
was 2.5 at that time. Lupron had ceased being effective. Bicalutamide had a terrific response.

Cancer return alert without scan or PSA ?
I'm more curious than anything to hear if you knew the cancer had returned somehow before any scan or blood test really told you? Pain? Bladder issues? Other signs ??? I'm wondering how many really knew somehow or sensed it and it was later confirmed ? Most people are very in-tuned to their own body

Treatment Plan
To update my journey, diagnosed on Oct. 2023 with pc,
psa
of 24.6. PSMA scan showed one met to sacrum, everything else clear. Did 28 rounds radiation and am currently on Lupron and Xtandi. Urologist yesterday said Lupron for one year then off and watch
PSA
with new PSMA scan also.

So the odds we all read about... are they true?
He's ordered another
PSA
test in early June to see if the April rise was just a blip or if it was indicative of the ADT beginning to fail.

Thinking of taking an ADT vacation
Would the
PSA
start rising once my testosterone level rises naturally after I stop the Lupron jab? What does the current low
PSA
level means? ...that the cancer cells are asleep/dormant?

Update for kestrel5
Recent scans and bloodwork: no evidence of cancer, three years of undetectable
PSA
. Never say 'cured,' but I am enjoying a period of durable treatment effects.

Looking for medical oncologist at Memorial Sloan Kettering.
PSA
has remained 0.7 for a many years. Although I think Fox Chase Cancer center in Philadelphia is a fine hospital, I decided to switch to Sloan Kettering in New York for my next round of MRI and biopsy. Does anyone have any suggestions for a doc to see there?

3 month PSA post RT/ADT, low enough?
Am I jumping the gun or should I be concerned that this almost three month follow up
PSA
isn’t low enough? I’m tolerating ADT pretty well so I’m not opposed to extending the duration if necessary.
