Help on what to expect with this 6 month follow up?
He has been on

for the last year as well. My questions are: 1. What can we expect on this follow up. Will they just set up new scans to see where he stands? 2. What would be considered a success / failure.

I just began the Eclipse Clinical Trial and will be posting my experiences here. Please share yours too.
The criteria was that I had to be castrate resistant and failed at first generation
ADT
, as well as second generation
ADT
. Plus that I had to have a minimum PSA of 2.0 with two consecutive rises one week apart.

looking for advice.
He was also put on
ADT
for 6 months. PSA dropped to <0.05 and stayed undetectable until August 2021 when it rose to 0.13. We were told not to worry. Bloods in Feb 2022 showed PSA was 8.63 and 7 days later 11. Ct scan showed numerous bone mets in ribs, hips and spine.

Google, I understand what they are, and that they are typically seen in old ladies with osteoporosis (not so different from this 70-year-old man on
ADT
). But I also see that it's been know to occur in men or women following pelvic radiation, which I finished 4.5 months ago.

Treatment: Unfavorable Intermediate Risk PCA
I could wait a month for the results of a Decipher test to determine whether
ADT
is needed but the Doctor is 80% confident it will indicate higher risk disease and the need for
ADT
. Therefore, I am leaning toward starting the
ADT
soon rather than waiting for the Decipher report to begin.

An Important thing to know if considering Estrogen Therapy.
I was also accepted into a phase 1 trial and later told that I must go back on an
ADT
treatment for 6mo before they would accept me.

Watching the results of the EMBARK trial with great interest as they had a ‘Lutamide monotheraoy’ arm that has performed very well and now questions are being asked about needing

for high risk of BCR in the hormone sensitive.

Orchidectomy?
And there are still many of the same SE’s that are experienced with
ADT
. Aside from avoiding the injections are there other advantages of getting one? Are there certain situations that make an orchidectomy a preferable treatment choice?

Heart-healthy foods.
Even before
ADT
. Mark Moyad coined the phrase "heart-healthy is prostate healthy" and I think he is correct. The American "food pyramid" is a disaster imo, with grains at the bottom & fats at the top.

ART How to Increase TET Naturally; Dr Rena Malik, M.D.
I am coming off 21 months of
ADT
and at month 6 my TET is 185! Sounds low but I am thrilled to high heaven...thought it might never come back. At month 5 I suddenly noted I had hair on my arms and 'low and behold,' some on my chest!

Why do we stop treatment
This is what I experienced and at the time I made the decision to stop the
ADT
, I was not informed. Like I said I had no idea what I was up against. I was more concerned with the side effects than I was with the disease.

Hormonal therapy and pain
Do you have experiences that hormonal therapy (
ADT
, Erleada etc) cause pain? What kind is it and have you stopped drugs? Or have you had a break and started it again? Or what has helped? My dad has been on
ADT
and Erleada for couple of months.

thoughts on virtual second opinions
His current MO has him on a zoladex injection and next
ADT
has yet to be added. I was looking into getting a second opinion at MD Anderson, Mayo and Sloan Kettering. I noticed that all offered virtual second opinions.

HELP! Firmagon reaction?
I only have two injections left and if it is a Firmagon issue, I'll want to change my
ADT
drug in 30 days. Please let me know your thoughts, I don't want to go back to the ER, they just aren't familiar with Firmagon. Hope this was clear, let me know if there are any questions. Thanks!

My story of my 23 year battle with Pca.
The first is diet, I would have eaten less red meat and processed foods. 2nd, I would have started
ADT
summer of 21 and possibly spared myself those months on crutches and 3rd I would have started supplementing with curcumin and magnesium many years ago.

Need to make a decision and I’m absolutely frozen.
But lots must be sacrificed for that peace of mind including that you won’t be able to chase the cancer (except with
ADT
) for possibly over a year if your PSA is abnormal post-surgery.
