You could ask for an MRI of the lower pelvis and muscles there. A sonogram generally cant see those areas (nor a laparoscopy or laparotomy) and pain in those areas can refer to the groin. I had an infection and was only treated with tetracycline for 5 days (many years ago). Then the terrible burning started on the lower right that responded to antibiotics. Sonograms and a laparoscopy saw nothing and grew nothing (but there was some inflammation). I kept saying that the pain would return after I went off the antibiotics. Doctors said that the only way that could happen is if it were an abscess - and I’d be dead if it had been an abscess. But they don’t tell you that there are infections (like TB) that can’t be determined by standard inflammation blood tests (sed rate and C reactive protein) and that can cause abscesses. They don’t tend to look laterally from the cervix even though blood supply connects all those areas. Later they found signs of infection in my lower right groin and cultured for TB but nothing grew. Many years later an MRI neurogram found a 2 inch long wound in my right obturator internus muscle with fluid in it. These are the areas where the pudendal nerves are. Over the years they tried steroids, painkillers, gaslighting me, but the only thing that has ever made a difference is antibiotics. Now I’m doing very well on long term antibiotics. Just be aware of the muscles of the lower pelvis. Based on my experience, infections can go there - and Drs don’t tend to look there. Good luck!
