Frequent urination (M, 24)

Frequent urination began in June 2023. I currently urinate more than 10 times daily. Do not drink coffee, tea, or alcohol. Clear urine, no pain, no blood, no burning sensation, normal flow. Recently started waking up once a night to urinate. Yellowish jelly-like lumps in seminal fluid. Unknown onset. Ejaculate has a liquid consistency. Prior to the onset of frequent urination I had recurrent right eye infection that was linked to stress and disappeared after 3 months. Blood tests for autoimmune diseases: Negative.

Accompanying Symptoms:

Lower back pain when lifting heavy objects. Occurred at the same time as the frequent urination.

Old poor hygiene habit:

Using the same toilet paper to wipe the penis head after defecation.

Sexual health:

1. I have had multiple sexual male partners (more than 10 in the past 2 years)

2. Received fellatio without protection

3. Negative tests: Chlamydia, gonorrhea, HIV, syphilis, hepatitis B and C (April 2024).

4. Received vaccines for HPV and Hepatitis A and B (April 2024)

First urologist (September 2024):

Ultrasound with a full bladder:

1. Identified prostate gland calcifications (likely due to chronic infection).

2. Prostate gland size: 20.73 cm³ (normal, smooth, elastic).

3. Bladder: Smooth outline.

4. Blood test for chlamydia: Negative.

5. Ejaculate test for Trichomonas vaginalis: Negative.

Second urologist (September 2024):

Ultrasound with a partially filled bladder:

1. Slightly thickened bladder wall.

2. Prostate volume: ~10 mL. Does not mention prostate calcifications

3. Kidneys: Normal, no drainage issues.

4. Urine analysis: Sediment and chemical tests: Normal, no bacteria.

Diagnosis:

Undetermined urinary tract dysfunction.

Treatment for overactive bladder:

1. Vesicare 5 mg (solifenacin succinate). Stopped after 2 months due to severe acid reflux.

2. Supplements: Various, including Saw Palmetto, Lycopene, Selenium, Enotprost

Minor improvement in urine retention ability during the first days. No lasting symptom relief.

Current Status:

1. Feel the urge to urinate approximately every hour or less.

2. Moderate or small urine volumes.

3. Difficulty fully emptying bladder when delaying urination.

What is likely causing this condition?

What other examinations can be made, like PSA?

Is this curable or just symptoms can be managed?

Don’t take this the wrong way, have you been using Ketamine?

Hi! No, I haven't taken Ketamine at all. I should have mentioned that I started taking high dosage of Vitamamin D3 supplements in November 2023. I stopped taking them in June 2024 if that could possibly have any effect even though my symptoms started before taking Vitamin D3.

Have you been tested for diabetes?

I have not been tested for diabetes. However, I wore a continuous glucose monitor for 2 weeks in May this year and the overall range of my blood sugar was 5. I had a couple of episodes of low glucose during the night that once reached 2.9. Yet, I noticed that when I was physically active, the ranges of the glucose normalised at night as well.

you would be better having an HbA1c test rather than do-it-your self assessment. Hypo levels of blood sugar are not normal!

You were well to stop such high levels of Vitamin D supplementation. Why did you start? Vit D is not a true vitamin and the body does not get rid of excess as it does with other vitamins.

Alright! Thanks for the advice. I started taking vitamin D because at that point I was living in Scotland thinking that I would become deficient in vitamin D so I started taking it. I was also influenced by my friends who were taking such supplements as well.

Yes, you can have too much vitamin D. That aside, your prostate volume is 10ml, was that judged to be ok??

They said the prostate was normal. The only thing was the slightly thickened bladder wall. I felt I wasn't taken seriously as they mentioned that these symptoms weren't typical for my age. Yet, this condition has lasted over a year now without improvement.

That is very comprehensive...

The prostate calcifications could be a result of chronic prostatitis, which can cause frequent urination and discomfort. The thickened bladder wall seen in the second ultrasound suggests overactivity or irritation, and may due to prior infections or chronic inflammation. Another possibility is strained muscles around the bladder and prostate which can mimic urinary urgency and incomplete emptying.

Have you had uroflowmetry and PVR to assess bladder emptying and urine flow strength?

Whether the condition is curable depends on if it stems from an infection or irritation or if it relates to structural or neurological issues. Maybe you should consult a urologist with expertise in complex urinary disorders or at least seek a third opinion...

Hi! Thanks for the comment! I haven't had uroflowmetry and PVR. I look up the options you suggested, I just really hope this is something treatable.

I’m the same as you but 30 years older although my symptoms began in my late 20’s, I’ve had similar tests and I got as far as prostatitis and over active bladder, my consultant has advised retraining bladder because of frequent urination, doctors dismissed prostate problems initially due to young age but after telling them my father had prostate cancer at 37 I’ve had psa and Dre tests every year since 30
