New here - Constant penis discomfort for 5 months, Dr. has absolutely no idea what's wrong!

Hello all,

Thanks for taking the time to read my post. It's a bit of a strange story so bear with me. I had unprotected sex one day and woke up with a large amount of penis pain, burning when I pee, all of that. So I went to the clinic and was treated for Chlamydia and later for Gonorrhea. After this treatment, the burning during urination completely stopped. I didn't have any discharge throughout this time.

BUT! I have had slight discomfort in the tube of my penis ever since! I wouldn't characterize the pain as "severe," but it is definitely constant, and is really frustrating. All sexual function is normal and I have been tested several times for any infections and there are none. My Doctor has given me several different antibiotics, including a cream, and none of them have had any effect at all. Just constant pain/discomfort.

It is definitely worse if I masturbate or touch my penis, and I sometimes feel like the pain will travel down to my legs and feet if I touch my penis. It is extremely strange, the Doctor has no idea what is going on, and it has really started to take a toll on my mental health!

Do you guys have any idea what's going on?

So were your tests for chlamydia and gonorrhoea positive ? and you went back for retesting after that time, did you have a swab inside your penis and have microscopy done ? if you are in the UK ?

It was actually in China, where I currently live. The doctor didn't do any tests at all, and just listened to my symptoms and gave me the treatment, haha. Because the burning when I pee feeling went away after the treatment, I assumed the pain would just go away on it's own, so it took me about a month to get it checked out again.

At that point, I went to a different (much better) doctor who is a proper urologist and we have been doing a urinalysis basically bi-weekly. The tests at this point are completely normal, and there is no sign of infection, but the pain still persists.

So regular urinalysis will not show the presence of STIs at all. Urologists do not do the intra-urethral test I mentioned - only sexual health docs do here in the UK. If you cannot get this test done, I would try to get mycoplasma genitalium testing as the antibiotics for ct/gc do not cover for this.

Also I am presuming that if the unprotected sex was a regular partner, they were screened or treated ? if it was a causal contact, have you had any further unprotected sex since then ?

Wow, again, thank you so much for the detailed and thorough response. Initially, I was treated with one dose of azithromycin and a shot in my bum, which I'm not sure what the shot was.

I will look into getting that test done. It was a one night stand. And actually I did use a condom for like 95% of the encounter. My penis slipped inside of her for a brief moment accidentally before I stopped and put on a condom. Since then I have not had any unprotected sex, or sex at all for that matter.

The urologist, over the course of several months, gave me 3 different types of antibiotics. These include Cefixime, Ciprofloxacin, Moxiflocacin, and Mupirocin.

The shot was likely Ceftriaxone which is the standard GC treatment. If you had mycoplasma, Moxifloxacin should have treated it - in the UK there are high levels of azithromycin resistance in mycoplasma organisms The standard course for moxifloxacin would be ten days. The brief contact you had is unlikely to transmit an STI but not impossible. Due to increasing antibiotic resistance globally , we only use antibiotics when there is a definite infection - which hasn't been proven in your case . And we often see men who are very anxious about infections , have many symptoms, but every test under the sun is negative.

So, any idea of the cause of the pain? My latest idea (obviously I am not a doctor and have no formal knowledge of this stuff), is that I have some kind of muscle issue, and if I stop masturbating for a while it could improve, as the only thing that I have found that directly affects the pain has been masturbation.

It is just quite depressing and odd, because I have no idea what the root cause is here. The doctor is going to check my prostate in a week and half, so I aim to not masturbate for that time and monitor the situation.

Any thoughts?

I would think it this was an infective cause, that all those antibiotics would have treated it . I would then be looking at urological causes. Youre already seeing a urologist - I cant suggest anything else I'm afraid .

What you describe sounds identical to symptoms I had for over 8 months continuously. Kept me up most of the night. The only thing that gave me slight relief was Ibuprofen. The big medical facility I was going to tested me three times for UTI at various times over a six month period of time.. The tests results came back in about 30minutes each time. They all came back negative. They kept telling me they did not know what the constant burning pain (that was ten times worse during urination) was caused from. After about 8 months the pain was getting unbearable. I went to a small local clinic near home and gave a urine sample. They did some kind of culture to test for UTI. The results took 3 days to get back and was positive for UTI. The little clinic gave me 5 days worth Cipro (anti-biotic) and have had no pain since.
