Anyone know if this is a genuine Disabled ID Card please?

Thinking about applying, as it would be so useful in so many ways for me, it is called the "National Disabled Identification Card (DID)".

Worried in case it is a 'scam' etc, as have to send personal details of proof of disability and eligibility. Anyone help please?
