Are you in the UK ?. You can get signed off on long term sick by your doctor if they think it is the right thing to do but you'll have to go for an ATOS assessment where you more than likely will be found well enough to work. I knew someone who was on the sick due to mental health problems and they were asked in by ATOS and didn't go because they couldn't and their claim was stopped.

I'm having the same issue! I just got hired at a new position with really good pay. I'm going to give it another try and if this doesn't work, my psychologist said I should try applying for disability. The last job I was hired at, I had to quit because I was having such bad panic attacks whenever I would have to get on to the highway to go home to the point a family member would have to drive to meet me and follow me home. So weird. But if you're in the US and getting treatment... the answer is Yes. Speak to your psychologist.

Don't let your fears rule your life, life is tough and you've got to beat it. I know it's hard but drive on the highway enough and you'll gain confidence and it won't bother you. Same with everything, I'm dammed if I'm going to let fear rule my life !.
