How much Levo are you currently taking

How long have you been on this dose

Was test early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

see contact GP for next 25mcg increase in Levo

Retest in 2-3 months

Likely to need further increase after that

What vitamin supplements are you currently taking

Folate is deficient

Vitamin D insufficient

Thanks for your help and reply.

I’m currently in 150mg levothyroxine. Do you think I should consider asking to go in T3/T4 combo?

My vit D went as low as 14 a few years and my GP put me on D3 400iu. But I recently found out from my cousin in a similar situation that that NHS guideslubes say way more. I will take a look at recommended sources you recommended.

I gurgit to mention that I had a endoscopy a few years ago which showed I have an hiatus hernia so I’m on omprazole.

Until recently, I was only taking them as needed but now I’m having such bad issues, I need to take them everyday. Im having bad diaroehha, bloating, nausea. Im find eating veggies, dairy ( I’m lactose intolerant, but could tolerate a small amount) Now, I can’t even tolerate a small amount in my coffee.

I was taking 2,000 magnesium glycinate but ran out and forgot to restock.

So you think my numbers fir anti bodies are ok?

As you are lactose intolerant you need to ALWAYS have lactose free levothyroxine

Lactose free brands - currently Vencamil or Teva

Vencamil is lactose free and mannitol free. originally only available as 100mcg only, but 25mcg, 50mcg and 75mcg tablets became available Sept 2024

Prior to March 2023 Vencamil was called Aristo

Vencamil often very well tolerated/best option for many people

Teva makes 12.5mcg 25mcg, 50mcg, 75mcg and 100mcg

For some people Teva is the best

But

Many patients do NOT get on well with Teva brand of Levothyroxine.

Teva is lactose free, but contains mannitol as a filler instead of lactose, which seems to be possible cause of problems. Mannitol seems to upset many people, it changes gut biome

Helpful post about Teva

Relatively new ……Hillcross brand

This is a box, rather than a brand. 50mcg and 100mcg are Accord brand….but be aware 25mcg is Teva brand

My vit D went as low as 14 a few years and my GP put me on D3 400iu

Very poor care from GP

Guidelines are quite clear

Vitamin D under 25nmol and you should have been prescribed total of 300,000iu over 6-8 weeks

panmerseyapc.nhs.uk/media/2...

And retested at end of that with advice to supplement continuously

Aiming to maintain at least over 80nmol minimum

Test twice a year. You may need higher dose in winter than summer

Previous post here with vitamin D info and dose of Levo

healthunlocked.com/thyroidu...

Buy yourself some vitamin D mouth spray and increase dose to 2000-3000iu per day

As you have MS it’s especially important to maintain good vitamin D

mssociety.org.uk/research/e...

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

leedsformulary.nhs.uk/docs/...

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

mm.wirral.nhs.uk/document_u...

But, improving to around 100nmol by self supplementing may be better

pubmed.ncbi.nlm.nih.gov/218...

vitamindsociety.org/pdf/Vit...

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Test twice yearly when supplementing

Can test via NHS private testing service

vitamindtest.org.uk

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function.

There’s a version made that also contains vitamin K2 Mk7.

One spray = 1000iu

amazon.co.uk/BetterYou-Dlux...

It’s trial and error what dose we need, with thyroid issues we frequently need higher dose than average

Low folate

GP should prescribe folic acid

Once that prescription finishes look at ongoing daily supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance

Difference between folate and folic acid

healthline.com/nutrition/fo...

B vitamins best taken after breakfast

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need 2 per day and/or may need separate methyl folate couple times a week

Post discussing different B complex

healthunlocked.com/thyroidu...

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 3-5 days before ALL BLOOD TESTS , as biotin can falsely affect test results

endocrinenews.endocrine.org...

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg)
