Help please... recent bloods.

Good morning. I am hoping some kind person will have a look at my recent blood results and help me interpret them before I see my GP.

Serum TSH - 0.286 (abnormal) Normal range 0.35-4.94

Serum Free T4 - 14.0 pmol/l (normal range 9-19 pmol/l)

Serum Free T3 - 4.0 pmol/l (Normal.range 2.4 - 6 pmol/l)

Plasma Vitamin B12 - 128 ng/ml (adnormal) normal range 51 - 128 ng/l Serum Folate - 8.3 ng/ml (normal range 3.1 - 20.5 ng/ml)

Serum Ferritin - 73 ug/l (normal range 22 - 204 ug/l)

I have been under active thyroid for approximately 7 years now. My current dose of thyroxin is 100mcgs daily.

I supplement with B12, zinc, selenium, vitamin D and cod liver oil daily. I also take atorvastatin 10mcg daily and results for that are normal.

I last medicated 24 hours before my bloods were taken.

The last few months I have been feeling irritable and moody and quite low. Tired and also experiencing hair loss.

My last bloods were in 2023 ( I hadn't realised it had been that long plus I dont have the best surgery and they never remind me) This is first time since diagnosis they have checked T3 and T4.

Any help would be much appreciated.

I use the mercury pharma brand of thyroxin.

Hi! Can you add the range for the free t’s. It’s possible they are on the low side but we need the range to know.

We need ref ranges to be sure, but I suspect you have a T4 to T3 conversion issue....high FT4 with low FT3.

If FT3 is low any number of symptoms can result....ĺike yours!

Your cholesterol has been high....again the consequence of low T3

I suspect it's T3 you need not statins....my cholesterol level dropped when my T3 became optimal....this is normal.

For good health every cell in the body must be flooded with T3 by way of an adequate and constant supply

Low cellular T3 = poor health!

So post the reference ranges. then we can check actual hormone levels and conversion status, then we can see if you might benefit from a little T3

OK....thank you for ranges.

FT4 is 50% through it's ref range

FT3 is 44.4% ditto

Your dose is 100mcg daily

Both Frees are too low

You have plenty room to increase your levo

Your T4 to T3 conversion looks fine

Suggest you increase levo by 25mcg....to 125mcg

Test again after 6 weeks on a steady dose of 125mcg.

Next move depends on that result.....if Frees are still low then increase to 150mcg...you should still have room to do this if necessary

We aim to have both Frees sitting at roughly 75% through their respective ranges

But, the main aim is to relieve symptoms so thay must be included in evaluaion

Your medics will likely bang on about TSH this is not a reliable maker, it fluctuates throughout the day

FT3 followed by FT4 are the important results

Caveat.....

Accurate testing depends on...

Test just before 9 am

Test 24 hours after last levo dose

Test on an empty stomach....except water

Following that protocol every time you test will give the most accurate labs.

Optimise...

Vit D, vit B12, folate, ferritin and antibodies Tg and TPO if not already tested.

Vit D:

The Vit D Council, the Vit D Society and Grassroots Health all recommend a level of 100-150nmol/L (40-60ng/ml), with a recent blog post on Grassroots Health mentioning a study which recommends over 125nmol/L (50ng/ml).

Vitamin D should be taken with vit K2, the latter helps calcium reach the hard tissues (eg bones and teeth) and away from soft tissues (eg arteries) I like the Better You spray which combines both

Vit D is a fat soluble ....so avoid overmedication!

Fat-soluble vitamins get stored in fat tissues and the liver. Your body squirrels away any excess you consume and doles it out gradually to meet your needs. Because these vitamins are stored for long periods, however, dangerous levels can build up.

Vit B12:

"We believe that the 'normal' serum B12 threshold needs to be raised from 200 pg/ml to at least 450 pg/ml because deficiencies begin to appear in the cerebrospinal fluid below 550".

According to an extract from the book, "Could it be B12?" by Sally M. Pacholok:

"For brain and nervous system health and prevention of disease in older adults, serum B12 levels should be maintained near or above 1000 pg/ml."

Vit B complex

We are advised to take a good Vit B complex I like Igennus Super B Complex

Vit B is water soluble so excess just produces expensive urine!!

Folate:

Folate is recommended to be at least half way through range

Ferritin:

Optimal ferritin level for thyroid function is 90-110ug/L

Serum Free T4 - 14.0 pmol/l

Serum Free T3 - 4.0 pmol/l

We need the ranges on these results

Different labs have different ranges

Ranges should be on results page

Most common range for Ft4 is 12-22

but we see wide variety….7-17 or 9-19 etc etc
