Your advice please-? indication for T3

Hi there, I am on Teva Levothyroxine 75mcgs after being diagnosed with Hypothyroidism just over a year ago. I am feeling better but still have episodes exhaustion. Following really helpful advice advice on a previous post I have just got my private blood test results back, taken on Monday this week:

TSH-3.8mu/l (normal range 0.27-4.2mu/l)

FT4-18.6pmol/l (normal range 12-22pmol/l)

FT3-3.3pmol/l (normal range 3.1-6.8)

TPO-11iu/ml

Vit D - 64nmol/l (normal range 62-102)

HB - 144g/l (normal range 124-270)

Ferritin - 90mg/l (normal range 10-405)

TSAT-27% (normal range 16-50%)

Active B12 - 48pmol/l (normal range 40-210)

Folate - 7.5ng/l (normal range 3-25)

I would be really grateful for your thoughts. Is T3 indicated or do I need to get my vitamins in order (I am only taking Vit D3 10ug and Omega 3, plus HRT?)

Hello and thanks for your post, your conversion of FT4 to FT3 looks poor. T3 might be the answer but you probably need a slight increase in T4 first.

Vit D on the floor aim for 100 it will make such a difference. You need D3 with K2 and a loading dose for 3 months of at least 4000iu probably more.

Ferritin could be a smidge higher, are you veggie or vegan? Try liver pate twice a week or similar heme iron rich foods.

B12 very low, I’d suggest asking GP to test for Pernicious Anaemia, very good forum on Health Unlocked, after testing then likely high strength B Complex and separate B12, maybe even injections

Shows you currently have room to increase dose a little

Very poor conversion rates

Are you on Teva because you are lactose intolerant?.

Suggest you change to Vencamil Levo at next prescription.

Teva upsets many people

Vitamin levels all very poor

Vitamin D ….increase dose to 3000iu and retest in 3-4 months

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

But, improving to around 80nmol or 100nmol by self supplementing may be better

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function.

There’s a version made that also contains vitamin K2 Mk7.

It’s trial and error what dose we need, with thyroid issues we frequently need higher dose than average

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement

A week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need separate methyl folate couple times a week

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 5-7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

Don’t make all changes at once ….only one change at a time then wait 10-14 days to assess before making further change

A) increase vitamin D and include vitamin K2. Plus add a separate magnesium supplement

B) add separate B12 supplement and week later daily vitamin B complex

C) look at increasing Levo to at least 87.5mcg daily

D) consider changing from Teva brand

E) get ultrasound scan of thyroid

F) Gluten intolerance is often a hidden issue too.

Request coeliac blood test BEFORE considering trial on strictly gluten free diet

Are you taking HRT tablets or patch?

HRT tablets (and vitamin D tablets) should be at least 4 hours away from taking levels

When did you start HRT

Retest thyroid and vitamin levels on 2-3 months after all changes done
