Hi everyone

I posted a couple of weeks ago and was advised to have bloods done. These are my results so hoping for further advice in supplementing or adjusting doseage. I currently take 50mcg Levothyroxine and 25mcg T3 daily. I also have B12 injections every 12wks. Latest B12 injection given day after bloods sent to Medichecks. Also on HRT both estrogen 1.5mg sandrena daily and 5mg testosterone daily

Just want my hair to stop falling out and to feel better.

TSH 0.01 (0.27-4.2)

Free T3 7.3 (3.1-6.8)

Free T4 10.6 (12-22)

Thyroglobin Antibodies 74.1 (0-115)

Thyroid Peroxidase <9 (0-34)

Inflammation 1.83 (<3)

Ferritin 146 (30-518)

Folate 16.2 (<7)

Active B12 >150 (37.5-188)

Vit D 84.2 (50-250)

presumably your 25mcg T3 is self sourced

Is GP aware that you are taking T3

Do you normally split T3 as 2 x 1/2 tablet or

Day before test did you split as 3 doses per day (1/2 plus 2 x 1/4)

How long have you been on T3

Did you build up slowly

How much levothyroxine were you taking BEFORE adding T3

What vitamin supplements are you taking

Yes T3 is self sourced and GP aware of doseage.

I take in whole form once a day usually (as per GP) but day before test I split into 3 x doses

Been on T3 since covid (was on NDT before then but couldn’t source so had to switch) didn’t build up slowly as swapped to what I thought was the equivalent (took 27mcg T3 in form of NDT. Take 25Mcg T3 now)

Can’t remember levothyroxine doseage before T3 as was on NDT for a few years prior and only switched back to T4/T3 due to supply issues in covid

Only take 1000mg VitC daily

Results suggest

A) you need to reduce T3 by 1/4 tablet per day as Ft3 is too high

B) increase levothyroxine by 25mcg per day as Ft4 is too low

Request GP prescribe extra Levo

Which brand of levothyroxine is 50mcg

Ideally get same brand

Or cut a 50mcg tablet in half to get 25mcg

Retest in 8 weeks

May need further increase in Levo after this

Approx how much do you weigh in kilo?

Thanks SlowDragon. I will do that. Always get the same brand of levo which is accord. I weigh approx 57kg

So if ONLY on levothyroxine guidelines suggest 91.2mcg per day as likely daily dose

That’s approximately 638mcg per week

Perhaps 75mcg per day three days per week and 100mcg four days per week

Accord don’t make 25mcg tablets

On levo plus T3 …..typically dose Levo is reduced by 25mcg per day

Currently your Ft3 is too high, so reducing T3 dose is first step

Thanks for all your help and advice SlowDragon I really appreciate it. It’s really hard trying to balance everything when there is no support. So I can put my case over to GP do I ask for increase of 75mcg p/day of Levo first plus reduce my daily T3 to 20mcg from 25? Then retest after 8wks? Or stay on 50mcg levo and just reduce my T3 to 20mcg?Do I need to a take Vit D supplement as well as a B complex? All I want is to not lose anymore hair

I would ask GP for increase in Levo

In consultation with GP …..say you will also be reducing T3

Yes I would reduce T3 first ……perhaps wait until after 4-6 weeks to increase levothyroxine.

many people on Levo plus T3 find they need BOTH Ft4 and Ft3 approx 60-70% through range

BEFORE considering adding T3

Typically dose Levo is increased slowly up to approximately 1.6mcg per kilo of your weight per day …..usually Ft4 would be around 70-80% through range

All four vitamins at optimal levels

Only then would you consider adding T3

Do you have Pernicious Anaemia that you are on B12 injections?

Or are you vegetarian or vegan

As you have B12 injections it’s recommended also to supplement a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance

B vitamins best taken after breakfast

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

Or

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) .Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg)

So your hypothyroidism is likely also autoimmune despite negative thyroid antibodies

Are you on gluten free or dairy free diet

Yes. When I was first diagnosed I had high antibodies. I follow GF diet and have done for approx 7yrs. Still have some dairy but usually have lactose free milk
