Hi, I’ve just had my test results back if anyone could help me determine if I have thyroid issues please I’d be very grateful x

Cortisol 331 nmol/L (The post 250 mcg Synacthen cut-off is >420 nmol/L)

Serum free triiodothyronine level 4.0 (3.1 - 3.6)

Serum free T4 15.9 (10.5 - 22.0)

Serum magnesium level 0.89 (0.7 - 1.0)

Serum TSH level 2.11 miu/L (0.27 - 4.2)

Thyroid Peroxidase Antibody < 28 iu/mL (0.0 - 60.0)

Looking at previous posts

Not clear are you now prescribed Levothyroxine

If yes, what dose are you taking

How long on this dose

Which brand

Was test done early morning, ideally just before 9am, only drinking water between waking and test and last dose levothyroxine 24 hours before test

What vitamin supplements are you taking

How do you feel

FT4: 15.9 pmol/l (Range 10.5 - 22)

Ft4 only 46.96% through range

Ft3 range not correct

Is it 3.1-6.8?

Hi, thanks for your reply. I haven’t started taking Levothyroxine as I wanted further tests done. I have hair loss and horrible hair texture which is always now breaking, fatigue, puffy face, split and broken nails, weight gain. I don’t take any vitamins. The test was done at 8.25 am with only water consumed. Yes, apologies, T3 range should have been 3.1 - 6.8. 😊

Next step is to test vitamin D, folate, B12 and full iron panel test for anaemia including ferritin

Low iron/ferritin often results in hair loss

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and add a separate vitamin B Complex after a week

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Igennus B complex popular option too. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

would suspect you might have Hashimoto’s

Previous post 4 months ago TSH over range even with test done at 6pm …..would have been higher at 9am

Serum TSH (tested at 6pm unfasted) 5.22 (0.27 - 4.2)

Serum free T4 13.6 (10.5 - 22.0)

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Get thyroid levels tested including both thyroid antibodies 6-8 weeks after adding vitamin B complex

FT3: 4 pmol/l (Range 3.1 - 6.8)

So Ft3 low at 24.32% through range

Likely low stomach acid with low Ft3 and then low vitamin levels as direct result

Hi, I had my vit D, B12 etc tested previously and the doctors said they were all normal. I think I did post them on here but if not I’ll try and find the test results.
