Hi, Here are the Blue Horizon test results for my friend where I think she's showing hypothyroid because of the low T4. Do you agree? Is Total T4 also a bit low in range, do you think?Next step will probably be to get the gp to test again and check the low T4 wasn't a one off as there's no thyroid panel history to compare to for her.

Her vitamins B12, folate and vit D are all usually and consistently deficient or low in range. Iron is usually high or good. So these deficiencies would fit with hypothyroidism.

Biochemistry

Magnesium 0.95 (0.7 - 1.0) mmol/L

Hormones

Cortisol (Random) H 673.0 (6am - 10am 166 - 507 nmol/L, 4pm - 8pm 73.8 - 291)

She did the cortisol test on waking up, early morning.

Thyroid Function

TSH 2.73 (0.27 - 4.20 mU/L)

T4 Total 97.5 (66 - 181 nmol/L)

Free T4 13.4 (12.0 - 22.0 pmol/L)

Free T3 4.65 (3.1 - 6.8) pmol/L

Immunology

Anti-Thyroidperoxidase abs <9.0 (<34) IU/mL

Anti-Thyroglobulin Abs 11 (<115) IU/mL

Iron is usually high or good.

Serum iron? Or Ferritin?

Hmm I'm not 100% sure but think it was ferritin. Is there a difference? Pretty sure it was ferritin on the blood test print out. One reading was high, above range. But then later readings were good, in range. Thank you!

Yes, there is a difference. Serum iron is the iron you have available for use by your body. Ferritin is a protein that stores iron for future use. It's a bit like a savings account, the body puts iron in and takes it out as necessary. So, your serum iron can be low, and your ferritin high, and verse visa.

However, you can get a false ferritin high if you have high levels of inflammation. And, to know that, you need your CRP - C Reactive Protein - tested at the same time as ferritin to get a better picture.
