don't understand why FT4 is so low
Your FT4 is low because you are taking NDT and it contains T3, the T3 lowers FT4, it's just what it does. Sometimes the ratio of thyroid hormones in NDT is not right for some people (ratio of T4:T3 is usually around 4:1 in NDT) and they are better off with separate synthetic T4 and T3 which enables them to tweak the doses to suit.
How do you feel on this dose? If you feel undermedicated there is room to increase as your FT3 is only 40.54% through range and most people on NDT or any combination thyroid hormone replacement tend to feel best when FT3 is in the upper part of it's range.
In what form of supplement is B12? Is it injections for B12 deficiency from your doctor? Or are you taking sublingual tablets because you originally had a lowish level but not actually B12 deficiency?

That is a very wide range for a female, that's usually the male range, female range is usually 13-150 or 13-300.
Ferritin is normally recommended to be half way through range but I do wonder in this case because, as I said, this is the male range so really it's difficult to say in your case.
Your ferritin may not necessarily low. First of all you'd need to test an inflammation marker, eg CRP, to see if you have any inflammation, inflammation or infection can raise ferritin. If CRP is nice and low then you have a true ferritin result and it seems it might be a reasonable level as some experts say that the optimal ferritin level for thyroid function is 90-110ug/L.
You can help raise your level by eating liver regularly, maximum 200g per week due to it's high Vit A content, also liver pate, black pudding, and including lots of iron rich foods in your diet
We shouldn't consider taking an iron supplement unless we do an iron panel, if you already have a decent level of serum iron and a good saturation percentage then taking iron tablets can push your iron level even higher, too much iron is as bad as too little.
Serum total 25-hydroxy vitamin D level 91 nmol/L;
This isn't too bad. The Vit D Society, Vit D Council and Grassroots Health all recommend a level of 100-150nmol/L with Grassroots Health's recent blog post recommending at least 150nmol/L. Maybe take something like 2,000iu D3 during the winter months when we can't make Vit D naturally from the sun. Retest in April to see if you need to carry on during the summer. When taking D3 we need it's important cofactors - magnesium and Vit K2-MK7.
I am self medication and found VitaliThy as gluten an dairy free. It is easy to buy.
I am feeling OK, but to be honest I can not see huge differences between be on NDT and synthetic. ( only one issue i had it was higher pulse) I have been on NDT 3 years and my weight was Ok, later I can't buy any more NDT I start using synthetic T3.

back to NDT and give myself 1 year to check.

So is your doctor giving you the B12 injections, for B12 deficiency? If so then you should continue. If you are arranging the B12 injections yourself, what is the reason, did you test low originally?
My witamin b12 was very low that why I had injection. Thank you for advice, I can not split my NDT, but maybe I will add synthetic T3 to increase little bit FT3.

What dose size does your NDT come in? Your FT4 is bottom of range, if you add synthetic T3 this will raise your FT3 level but will probably lower your FT4 level even further. If your NDT comes in half grain sizes then I would try an extra half grain, if only 1 grain size then there's room to add an extra 1 grain and see what that does to your levels and how you feel. There's room for both FT4 and FT3 to be higher.

I am curious if you are still on vitalithy. Were you on synthetic before and how did you covert the dosage...i was on 3 pills to equal 3 grains armour but felt like i was gaining some weight and have some joint issues wondered if undermedicated and should add a pill
