Good Afternoon,

I got some new test results here and was wondering from the wonderful informative people here would help make them out :)

Vitamin B12: / 396 pg/mL Range: 180-914

Ferritin: 56 ng/mL Range: 23.9-336.2

Folate: 19.1 ng/mL Range: >3.0

Vitamin D: 36.4 ng/ml Range: 30-100

Deficient < 20

Insufficient 20-29.9

Sufficient 30-100

Upper Safety Limit >100

TSH: 0.78 uIU/ml Range [0.45-5.33]

T4: 10.7 ug/dl Range: [6.1-12.2] ug/dl

Total T3: 95 ng/dl Range: 87-178

I just feel like something is still off... I do get brain fog it seems still.. I fall asleep but wake up every few hours every night! sometimes my fingers and toes will get cold... still got the dang ringing sound in my ears!!!! I do workout like 4-5 days a week though thankfully! I eat somewhat healthy! any supplements I should be taking from these numbers? my PCP doctor says I am in range... ugh! I have an appointment with my Endo next week!

Well, let’s have a look. :)

Your B12 could be quite a bit better. Many people find they feel better with a B12 level over 500. Would be worth supplementing for a while to get that higher.

Folate looks decent.

Your ferritin is lowish. Generally here, we suggest you aim for somewhere nearer the middle of the range (and definitely nearer 100). But—it’s important that you get a full iron panel to make sure that you do need additional iron. Sometimes low serum ferritin can be misleading.

Vitamin D is ok. Would be well worth letting some sunshine get to your skin to keep your levels topped up (I see you’re in the US so hopefully summer’s on the way where you are? :))

So on to the thyroid levels. TSH, as your doctor has cleverly observed :) is in range. But there’s wiggle room there for a dosage increase. Your FT4 also isn’t too shabby either. Unfortunately though, your doctor has ordered a total T3 test which unfortunately doesn’t tell us very much about how much of your thyroid hormone replacement you’re converting into T3, the active version of thyroid hormone. It tells us about both “bound” and “unbound” T3 as a total—impossible to tell how much of each. Measuring Free T3 (FT3) gives us a better idea of what’s available for your body to use.

Your T3 doesn’t seem to be especially high though, does it? Even if that is a total figure. Feels like you might benefit from a small dosage increase. Working out uses up T3 really quickly, so it does seem like that could be a good idea.

How much levothyroxine (T4) are you currently taking?

I am on 150mcg Levothyroxine Monday-Saturday and on Sunday I take 1 150mcg and a half of 150mcg

Transferrin: 237.00 / Range: 203-362 mg/dl

TIBC: 339 / Range: 250-450 ug/dl

Iron Saturation: 23.3 / Range: 20-50 %

Looking at profile, you have had RAI

How much levothyroxine are you currently taking

Do you always get same brand of levothyroxine

What vitamin supplements are you currently taking

Ferritin, B12 and vitamin D are borderline and likely need improving

Look at increasing iron rich foods in diet for ferritin

Are you currently taking any B12 or Vitamin B complex?

No B12 or Vitamin B Complex or Vitamin D supplements any specific dosage of each I should take?

Low folate and B12

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

Or Thorne Basic B is another option that contain folate, but is large capsule

IMPORTANT If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

With B12 result below 500, recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months, then once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

Yes, she’s our expert. As Slow Dragon advised, you could supplement with both B12 and a B complex - I see you have ordered the Igennus B complex. It does contain B12 so that may be enough for you, or, you do an additional short course of B12 to top up the B complex.

I’m not sure about your D result, I think our range is different to US.

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function. There’s a version made that also contains vitamin K2 Mk7
