Should I do more testing following results

Got thyroid test back blue horizon gold) not sure where to go from here, test taken on waking at 6 am

Cortisol 492 (6 am - 10 am 113 - 456)

Thyroid all in ranges

TSH 1.05 (0.27 - 4.20)

T4 total 84.3 (66 - 181)

Free T4 12.6 (12 - 22)

Free T3 4.85 (3.1 - 6.8)

Vit d 49 (50 - 175)

Rest of results were in range & no antibodies.

Dr comments mentioned saliva testing due to raised cortisol which can indicate cushings. Also vitd.

I was on vit d until sept last year when my gp stopped it as she said it was in range. Diagnosed with gastritis following endoscopy 2 weeks ago, fibromyalgia approx 5 years ago. Thyroid scan in 2018 showed normal size no goitre but fluid in neck.

I only take Pepcid for gastritis, no pain meds or supplements.

Symptoms are slow weight gain approx 2 stone, fatigue, joint & muscle pain and stiffness, (especially in lower legs) early waking insomnia, ringing in ears, debris behind right ear drum, (waiting to see ent) burning throat and mouth every morning, sore bleeding gums. Neck swelling at front (maybe fluid) I am 49 so probably perimenapausal too as periods very light.

Not sure if I should get cortisol checked privately, go to GP or try and get private endocrinologist. Any suggestions or experience of similar greatly appreciated

TSH is low

But Ft4 is very low too ....suggests you’re hypothyroid

Have you had cholesterol levels tested recently?

High cholesterol linked to being hypothyroid

Please add actual vitamin results and ranges for folate, ferritin and B12

Vitamin D is much too low

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

High cortisol levels common when hypothyroid as adrenals try to compensate for lack of thyroid hormones

Suggest you get coeliac blood test done

Assuming test is negative trial strictly gluten free diet 3-6 months

Thank you for your replies and suggestions, hopefully I’ve managed to attach full results now. My GP likes to put everything down to fibro so will see if I can get tested via blue horizon for cholesterol and coleic first. It’s taken almost 3 years to get endoscopy for heartburn

So your Ft4 is extremely low

Strongly suggests hypothyroid

When hypo we frequently have low stomach acid, this leads to poor nutrient absorption and low vitamin levels

Vitamin D - see previous reply re improving low levels

Also magnesium

Folate and B12 also on low side

Are you currently taking any B vitamins

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

Or Thorne Basic B is another option that contain folate, but is large capsule

IMPORTANT If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

With B12 result below 500, recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months, then once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

Pepcid is to lower HIGH stomach acid
