Cannot raise T3 dose but need to increase FT3 - could my T4 be the problem?

Hi everyone,

I have tried now multiple times over the period of the last 4 months to up my T3 dose but I just cannot tolerate it without horrendous symptoms. Even increasing by just 2.5mcg causes the same problems - strangely symptoms are not worse with 5mcg increase than with 2.5mcg. I clearly need an increase in my FT3 levels as last test result showed them 43% through the range and I feel very hypo still. When I remain on my original 10mcg T3 I get colder, cold hands and feet, dry skin, constipated, reflux worsens considerably, digestion slows and I also experience severe buzzing/vibrating. My weight gain (needed as still 1 stone underweight) has also stopped completely and sometimes drops which for me is a hypo symptom. When I increase T3 my digestion improves almost instantly but my heart becomes thumpy and the rate speeds up and down throughout day and becomes more erratic, I experience bad anxiety, lightheadness/out of body feelings, mild headaches, shaking, I am still cold (wearing long warm trousers, long sleeve tops and jumpers in the 26C heat!) waking an hour before I normally do feeling anxious and higher heart rate and worse fatigue.

When I first started on T3 my FT4 was 88% through range and I had low vitamins and ferritin but I had no where near as much trouble introducing T3 as I am now trying to go up by only 2.5mcg!

A few months ago when these bad symptoms first started to intensify after being better I thought I was overmedicated and dropped T3 and many symptoms intensified. I had a blood test that showed T4 61% and T3 26% through range and restarted T3 and felt better but my endo also upped T4 by 12.5mcg and I simultaneously had some of the issues above including higher heart rate. I am now wondering if I cannot raise my T3 because of my T4 level. I have a T4 75% through the range on 87.5mcg (so it actually increased 14% on only 12.5mcg where previously it went up by 10% per 12.5mcg). Previously back in march T4 was 73% on 75mcg and T3 was 70% on 10mcg and I felt the best I have so far in my recovery with no vibrations, more regulated pulse and body temperature and hugely improved digestion and vitamin levels (all since adding the T3) but I was not 100% and had bad fatigue and some lingering reflux. The entire time I have been on 87.5mcg I have felt bad - bad if I stay where I am bad if I try to up T3.

I also as known have central so before I started on T4 my TSH was already 1.8. TSH had never previously been measured during treatment as useless for central but at last test doctors did it and it was <0.03. I am wondering if I experienced the initial improvement from the T3 but now that my TSH has become suppressed my already poor conversion has decreased and so the high T4 is just not converting and sitting in blood causing my issues? Can that happen? Reviewing my daily notes the symptoms also started up roughly 7 weeks after I started 10mcg t3. Is this something that is probable or even able to happen? It just seems strange to me that I have never felt well even with T3 70% through range though other symptoms improved. I am also aware its more usual to have a lower T4 and a higher T3 when on combination therapy. Do you think it would be worth trying a decrease in T4 to see if it allows me to tolerate an increase in T3?

i dont knowthe answers here, but i note you said you felt better on 75 mcg levo and worse on 87.5.

I would be tempted to reduce the levo to 75 again. then wait a few weeks b4 deciding next action

Thank you for the response! That is what I’m thinking! Hopefully this will be the difference I need to be able to up t3!
