Spoke to doctor at my surgery this morning and has contacted endo team at hospital, as was worried, may be thyroid storm. Told to stop taking levothyroxine for a week ( have only taken 1 out of last 5 days as making me feel, sick and dizzy) and then they will restart again at a much lower dose as TSH is suppressed, T4 is too high and T3 is also high. Tried to explain about Hashimotos hyper swing but either doesn't know what that is or doesn't think that it exists. My main worry is that I will have a huge hypo crash and as my mental health has deteriorated rapidly over the last 3-4 weeks, my health anxiety is sky high. Any feedback would be helpful.

This is still on the low side. Are you sure your B12 supplement is 100mcg? That's more of a maintenance dose, the dose to raise a low level with a B12 stand alone supplement would likely be 1000mcg, or within a B Complex would be 400mcg.
If taking B12 we should take a B Complex to balance all the B vitamins. However, with your current level a good quality, bioavailable B Complex containing 400mcg methylfolate should now be enough, eg Thorne Basic B, without the need for a separate B12 supplement.

This is low and would be better in double figures. The B Complex mentioned above should help raise this level.

Are you supplementing D3? You should be at a dose of about 4,000iu daily, along with D3's important cofactors - magnesium and Vit K2-MK7, retesting after 3 months and adjusting dose to a maintenance dose once the recommended level of 100-150nmol/L has been reached.

Testing magnesium is unreliable. About 99% of magnesium is stored in bone, muscles and soft tissues, leaving about 1% in the blood. So testing what's in the blood isn't giving an accurate picture of our magnesium status.

Ferritin is recommended to be half way through range so this is a good result.

I'm not au fait with interpreting blood cortisol results but this seems low to me.

Most doctors don't know much about Hashi's or the swings it can cause. Your FT4 is gradually coming down. You are not likely to have a huge hypo crash with those results, the FT4 and FT3 would need to be much, much lower. I'm not a doctor and not giving advice but it would seem that staying off Levo a little longer might be the way to go, your symptoms should tell you when you need to go back on I would have thought, or persuading your GP to test weekly perhaps.
