Insurance cover for Belatacept anti rejection infusion therapy. How do I get insurance for this drug therapy? My Medicare ends in 3 months?

I am Allison. I live in Texas. I unfortunately became very sick in late 2014, bit didn't go to the Dr for a few months. I was hospitalized and diagnosed with stage 4 cirrhosis. Recieved a liver transplant Thanksgiving day 2016. Was doing well until at a regular 3 month lab ALP jumped from 150 to 450

I'ts been 2 years since my kidney transplant and along with my anti rejection meds I have been taking bactrim for the last 2 years and i have had no infections yet my doctor has not advised me to stop taking this med and i have heard that taking it unnecessarily long term can actually cause problems.
