augmentation on neupro+Gabapentin. should I switch to a different dopamine agonist? All that’s helping is cannabis.
I’ve been on Neupro 1mg patches for 4+ years. I also currently take 800mg Gabapentin: 400mg twice a day. In the last month my RLS symptoms have appeared nightly beginning around 9:30pm. I’m able to fall asleep but then wake up in the middle of the night with RLS symptoms. The only thing that’s helped

Parkinson's linked to higher levels of pheomelanin in brain: Could it be a biomarker or treatment target?
In a recent study published in Progress in Neurobiology, researchers discover that the substantia nigra of individuals with Parkinson’s disease had a higher concentration of pheomelanin, as well as pheomelanin markers associated with L-3,4-dihydroxyphenylalanine (DOPA) and dopamine, than healthy controls

My letter to all powers-that-be, following my desperation for urgent need for improvements in education of medics an in management of RLS
Dear curriculum creators and top docs, As a medic with published research on Restless Legs Syndrome (RLS), I raise a number of concerns and some possible solutions , which have largely been endorsed by ******* Education Policy, GMC, who recommends that I contact you with these concerns to enable change

Alternating meds
I wonder if anyone else has experience with a RLS therapy method of alternating .375 mg Pramipexole and Methadone 20-30 mg every 10 days? I also use marijuana which helps maybe 40% for about an hour. My case is very severe with many previous pharmacologic failures including Alpha 2 Ligands, Buprenorphine

Augmentation Concerns Being Overblown?
04/03/23 Edit : In hindsight, with apologies to those have already responded - though I think the body of the post communicates the concern better - I wish I would have titled this post “
Is the risk of developing DA augmentation overblown?

Current Status and Future Perspectives on Stem Cell-Based Therapies for Parkinson’s Disease
The loss of a specific cell type, midbrain dopamine neurons in the substantia nigra, is the main cause of motor dysfunction in PD. Numerous preclinical and clinical studies using fetal cell transplantation have provided proof of concept that cell replacement therapy may be a viable therapeutic approach

Using L-Tyrosine with Fluoxetine (Prozac)?
hello new member here. I’ve recently been prescribed Fluoxetine 20mg and was just wondering if it’s fine to take with 500mg L-Tyrosine thanks? After doing a bit of research it seems Fluoxetine increases serotonin levels in the brain but does absolutely nothing for Dopamine levels which is why I would

has anyone heard of Brillia and S100B protein?
I was reading about this in the ADHD community. Sounds interesting. I think there is some crossover with ADHD and Parkinson’s symptoms https://en.wikipedia.org/wiki/S100B https://pubmed.ncbi.nlm.nih.gov/33052436/ “The exact etiology of Parkinson's disease (PD) remains obscure, lacking effective diagnostic

Mayo dopamine agonists withdrawal procedure
Hi everyone, any comment on below? "Discontinuation of dopamine agonists because of severe augmentation or other adverse effects and substitution of a drug of a different class (such as an alpha2-delta ligand) can be achieved in 2 ways. The initial drug can be reduced slowly after the new agent

Thought I was figuring things out...
I had to move my psychiatric care to the VA, after so much difficulty with Moda and then Kaiser. I had been diagnosed with ADHD at the age of 47 (now 48) by a MD psychiatrist. The VA set me up with a psychiatric mental health nurse practitioner over telehealth. On our first visit, the NP said to me

Phellodendron - MAO-B - Dopamine - Relora
I was thumbing through the Feb LEF mag & noticed that phellodendron tree bark was being touted as an inhibitor of monoamine oxidase B (MAO-B). MAO-B is an enzyme which breaks down dopamine. Our levels of the 'feel-good' neurotransmitter decline about 13% per decade after age 45. MAO-B is also a target

Insulin Resistance, Parkinson's Disease, Consequences And What To Do About It
The topic of insulin resistance (IR) in PwP is not often discussed. Has your MDS or neurologist ever tested you for IR? It is estimated that two thirds of people with PD who are not diabetic, have IR and very likely are not even aware they have IR as discussed here : https://www.parkinson.org/blog/

What does GPT4 have to say about the role of inflammation in RLS?
GPT4 is the latest release of a language based artificial intelligence program and I asked it what role did systemic inflammation have in RLS. This was the answer I got: "Chronic inflammation can contribute to the dysfunction of the dopamine system by damaging the dopaminergic neurons in the brain

Placebo vs Nocebo
Everyone knows about placebo; You take a medication and feel better but the medication is a sugar pill. Nocebo is the opposite side of the coin; I feel worse after a trip to the doctor. I feel worse because he asks whether I have stiffness or bradykinesia or balance problems or tremor or

Parkinsons? Or not?
My husband Peter has had a cervical spinal cord injury for 11 years now. This badly affects his mobility, upper body and hand function. He suffers from spasticity and very tight muscles. He can walk with aids but uses a wheelchair outdoors. Some months ago he started getting dizziness/lightheadedness
