I've been struggling with personal problems (my dad is terminally ill, due to brain tumours), work and financial problems (he was self-employed, so I also have to work to guarantee that our income is going to be stable) and last, struggling with my depression, anxiety and ADHD at college, feeling that I'm so lazy, and feeling that my grades are strictly related to the sense of my self-worth.
The question is: how to deal with so much?

Welcome to our supportive community. Well done for taking a brave step out of your comfort zone to reach out for help. Hopefully, you will find folks here are only too willing to come alongside you and share from their personal journeys and experiences.
You are obviously having a difficult time right now, and what are dealing with is massive, and too much for one person to be carrying this load. No way are you lazy, I'm not surprised your college work is suffering a bit. Can you get help for your dad from his medical team or social care? Did your dad pay into any health insurance scheme that he could access? I am assuming you are based in the USA from what you are saying, so I don't know if you any State/Federal Benefits system you could access on his behalf, or for you as his carer, to help with some of the financial pressures.
In regards to College, can you talk to a counsellor or personal tutor about your situation, and get some help and support? Also, you need to look after your own mental and physical health. Ask your doctor for a referral to your local mental health team to access treatments and talking therapies, that could help you stay as healthy as possible.
