Estradiol levels high? and progesterone help!

Good morning!

I'm 47, and have been on HRT for about 2 years. I have evorel 50 and 2 weeks of micronised progesterone every 12 weeks.

I also have under active thyroid disease and have to have vitamin b12 injections, so some symptoms can get confused!

HRT has managed hot flushes and other peri-specific symptoms well. I have private blood tests for the other conditions, so out of interest more than anything had Oestradiol and testosterone tests bolted on. The testosterone came back ok I think, 1.29 nmol/l 0.29-1.67, free testosterone 0.012 nmol/l 0-0.13.

However, I have been surprised by the high Oestradiol result - 1493 pmol/l 222-854. I have been getting headaches and annoying weight gain. There are symptoms that could be put down to the other conditions, but include dry skin and bloating. Should I try reducing my Oestrogen, or could it just be a natural spike? I would be interested to hear peoples thoughts/experience with this.

Progesterone. I hate it. I'm reluctant to have a mirena coil as I have had issues with coils in the past. Initially I had the evorel sequi, the progesterone didn't agree with me, so I have tried micronised progesterone. Initially monthly, but I really don't tolerate it well, particularly with mental health, which is otherwise not an issue for me. So I now take it for 2 weeks in every 12. It's a horrible 2 + weeks. Last time I tried it vaginally at the 200mg/day dose. This was better but made me uncomfortably bloated.

My question is about vaginal dosage. I have read that the Newson clinic advises 100mg/day for 2 weeks, however I found the BMS guidelines recommend 200mg/day. I would be interested to hear thoughts on this please.

I have had the same issue with the progesterone - I have a post on here about this. I looked up the NICE guidelines, there are no references for MP protecting against EC, it's just assumed. The only longitudinal studies available conclude that Micronized Progesterone doesn't protect against Endometrial cancer. If you can be bothered to fight your corner, go to your doctor and get a referral to talk to a consultant, or go see someone privately and try and get them to just give you Estradiol without the Progesterone.
