ESA FORM

Hi every. Another question for you all? I have another ESA form to finish filling out its taking me ages as I cant write much at one time. Right as I sent one ESA form in in March with all letters from GP ect, have I got to send the same letters again with this form this time? Or have they still got those letters on record? I would assume they have nothing on record. Even assume when answering each question that they won't include information given elsewhere unless you restate it (if relevant).

hi personally i have found if you just re send it all (copies ) then they have verything there and no need to waste time going back and forth in post so do the whole lot love didle xx

I sent in everything again, mine had to be back by 3/4/12, l still haven't heard anything apparently there is a massive bag log, It only took a month the last time, but then they did cock the last one up grrrrrrr! xx good luck xx

Can you get CAB or DIAL to fill in the form with you present on your behalf as it is difficult for you to write, it may be helpful for you.

I'm not going to send in duplicates to them with another form I've had to fill in.

I usually have a welfare rights bloke to fill them in for me, but he couldnt fit me in until after the date form has to be in. So it will have to be a do it yourself jobby. But can only write for so long before pains in my hands. Thanks Guys x

Hi sadoldred,

The online interactive .pdf version is on this page, if typing is any easier than writing. You need to save the webpage to your pc first and then complete it.

I have it and other forms on my pc and if anybody wants/needs help filling in theirs, all you need to do is private message me with your phone number and we can do it over the phone.

I'll email the completed form to you so you can just save it on your pc, print it off and sign it. I will delete it from my pc as soon as you email me to let me know you have it, as I have no wish to keep any information about anyone.
