both standard rate and have know TMJ and leg pain and foot pain do I need to be assessed again as I was assessed in 2021 with 10points

Being diagnosed/suffering a new condition is not a change of circumstances. If you ring up about this you will be sent new forms & have to complete them & have another assessment.

Only if you feel you could change your existing award to a higher one, i.e. enhanced for either/both components, should you report this. Perhaps get some advice before contemplating this from here to see if it's thought you could gain those extra points: advicelocal.uk/ choose 'welfare benefits' from the drop down menu.
