Grade 3 Fatty Liver - Liver Enzymes keeps on elevating

Last month I was diagnosed with grade 3 fatty liver through an ultrasound. I am 27 & extremely obese ( BMI - 33 ) . Just before that I had a drug reaction which led to short term Hemolytic Anemia and Jaundice . At that time everything on my LFT was elevated . It's been 40 days everything else on my LFT is normal , i have changed my lifestyle. I have reduced 5-6 kgs of weight , I eat extremely healthy now but my SGOT/AST & SGPT/ALT keeps on elevating . They were 105/206 when I had jaundice and bad lifestyle but now they are 225/330 after 40 days . I have been following everything that should reduce this . Doctor had given me just one tablet ( ursodeoxycholic acid ) i have been taking that as well but they keep on going up . The Ratio is still normal though . Is this common in Fatty liver ? I also have T2 Diabetes but it's completely in control ( Hba1c - 5 )

Update : After one month of rigourous training , my ALT/AST Levels are finally coming down . i stopped Urso acid that day only and has been completely focusing on my diet. They are fully down but down from 225/300 to 65/120

I'm surprised you have been prescribed URSO for fatty liver that's usually prescribed for PBC

Ursodeoxycholic acid may increase liver enzymes. At the beginning of treatment with URSO, it is usually recommended to check ALT, AST and GGT monthly, and then every three months. If ALT and AST are constantly elevated in fatty liver disease and this is not associated with the use of URSO, then most likely we are talking about steatohepatitis (NASH) but not just NAFLD. There are certain biochemical integral blood tests by which doctors are able to judge the degree of hepatitis activity in fatty liver disease and and the possibility of liver fibrosis. Also drugs used to treat diabetes, such as metformin, can lead to an increase in liver enzymes in some cases. In any case, you should discuss with your doctor whether you should continue taking URSO if your liver enzymes are elevated.
