Fatty Liver severe Stage 3 no fibrosis

I have Lupus so my blood gets testing every three months in 2013 I was told I had mild fatty liver then I changed my diet and in 2016 I was told I no longer had fatty liver However since then I went from 5’2” 115 lbs to in 2021 149 lbs

I have been having abdominal issues for a little over a year since August 2020. I did a CT in February it said basically nothing was wrong. I did an ultrasound showed fatty liver in April. I did a colonoscopy in May had two polyps and esophagram which showed mild gastritis and small hiatal hernia. I did a MRCP in June showed fatty liver no staging. Finally did Fibroscan last week with CAP 364 stage 3 and 4.5kpa stage 0-1 no fibrosis. I take bloods every three months since Jan 2015 when I was diagnosed with Lupus and liver enzymes have always been normal ast low 20s alt high teens. Starting April of this year I started having fluctuating high ast and alt enzymes.

Under 40 for ast and alt is normal

April ast 53 alt 83

May back down to ast 37 and alt 39

July up again ast 43 alt 72

July ast 55 alt 85

August ast 36 alt 48

September ast 34 alt 36

Last test done 9/27 ast 57 alt 89 also ggt 67 over 60 is high tested this only once prior in April and it was 58

This last test prompted the ordering of the Fibroscan.

Doctor tested for autoimmune hepatitis, PBC, psc, multiple myeloma and some other things which looks to be negative. He then ordered the Fibroscan I guess they try to rule everything else out before looking at fatty liver?

Doctor ordered more blood tests which I will take in two weeks around 11/30.

Follow up appt 12/2 and again 12/7

I now believe all of my abdominal pain and fatigue was from fatty liver. If not for the pain and sudden elevated enzymes I would never know the severity of my fatty liver. I actually lost 23 lbs since April and have been eating pretty clean but Fibroscan still showed stage 3. It is my only Fibroscan so I don’t know if it went up or down since April. I’m hoping it is going down. I believe if I continue to eat clean and lose a little more weight I can reverse this. I am now 124 lbs and 5’2”. I think 110-112 would be good weight and just keep eating vegan. No salt. No sugar. Just clean fresh veggies some fruit and nuts that I can reverse this disease. At least that is my plan. If anyone has any inspiring stories of reversing severe fatty liver please share.

I also would like to know how long for me to reverse this from stage 3 since it has been 7 months of losing weight and eating pretty clean. I suppose it may take awhile since it took 2 years to reverse mild fatty liver. This is severe so I expect another year or two to reverse? The difference is now I am eating 100% clean no cheating and losing more weight. I was stuck at 129 for a couple months. I also have been taking fish oil, probiotics, and milk thistle for years. I stopped taking Omeprazole and pain meds. I think I may add vitamin e? I would like to know what others think about that. I also take a teaspoon of tumeric daily and drink green tea and dandelion tea.

Hi Lorelle

It seems like you have a good understanding of your situation. Lupus flares can easily mask liver issues so diagnosis is difficult but good diet and exercise are your best bets. Vitamin e is considered useful for many people with liver issues but discuss with your doc. As part of a liver friendly diet we suggest increased extra virgin olive oil and reduced vegetable seed oils and saturated fats, but walking the auto immune tightrope is always an individual thing. My doc told me to take vitamin E but didn't say what kind, what kind do you take? I have just regular vitamin E, is there a difference?

The vitamin E sold at your drug store is fine but here is some info

Vitamin D (D3) helps to manage (down-regulate) autoimmune disease. Keep eating plant-based. Drink coffee (3 cups a day), and meditate to manage stress. You'll get through this.
