Another Laparoscopy? Help!

Hey so some of you may know me and my story. For those that don't here's a summary; I'm 37, I've been diagnosed with Endometriosis in 2017 through my diagnostic lap, the specialist performed an excision around my bladder and uterus (stage 4). In 2018 I had a planned further excision of endo from my bowel and remove regrowth (stage 3). In 2020 my symptoms came back so I needed another op to excise endo from my bladder and uterus, also removing an ovarian cyst (stage 3).

I have undergone two failed rounds of IVF, both fresh (i.e. egg collection and transfer in the same round), one long and one short, different drugs. We're taking a break for a few months to have some normality. I'm concerned due the amount of drugs and short timescale that my endo may have returned, that combined with some of the symptoms retuning have meant we've gone back to my endo specialist to find out if another investigation and excision is required. He's not keen on performing another lap considering the regrowth may be minimal and there will be much more scar tissue as a result of the lap he could do. He feels the risks outweigh the benefits. So has suggested and prescribed painkillers for the times when it's bad, to just bear the pain ttc naturally and cope with the symptoms during IVF (if and when we decide to do that again). The pain however is awful. The next step he said considering my age and number of operations (only 3!) either if IVF doesn't work or we've just had a child to do a complete hysterectomy as that means we don't need to worry about post surgery scar tissue. He's said it's entirely up to us what we want to do.

We know and trust our endo specialist, so we know if we choose to have another lap he'll do it well. However he's said himself he's not a fertility expert so he cannot say the effect endo regrowth could have on further IVF or vice versa. I feel like having the lap will mean he can take a look inside and remove anything which has grown back either due to my own body or the IVF drugs, so that when we do go for our next round(s) I feel like I've done something. I'm just worried if scar tissue as a result of surgery is as big a concern as he says; surely any endo now present could hinder uterus growth and could also be the cause of my low numbers of eggs maturing for IVF. So I guess my questions to you, from your experience, and feelings what would you do? Is post operative scar tissue that much of a worry after only three laps? Is painkillers okay for the next few months / year? Should I consider a hysterectomy in the future as my only option? Are more laps actually okay? Would a different specialists opinion be a good idea even if they are not specialised enough to perform excisions?

The dreaded scar tissue! To be fair, I’m not your consultant, so I don’t know if there’s been an increase in your scar tissue or not…

But the risks are very real!

My first surgical lap was to treat scar tissue, along with stage 4 endometriosis, which was my last one, for 5 years! I’m now awaiting a hysterectomy, but because my scar tissue is advanced, far worse than my disease, I need 3 surgeons to get these organs out! My hysterectomy they’re reluctant to do, keeps getting put off!

Scar tissue left my dad inoperable! Scar tissue first caused a strangulated bowel, before they realised how bad he was! A hysterectomy will not cure scar tissue, unless it only effects those organs!… You cannot cure scar tissue!….

So yes, believe your gynea and think carefully about your next steps!

If planning kids, you need your ivf to fall pretty fast after another lap if you decide that route! Xx
