If you have most of these symptoms, you can just write for example all but 1, 5, 12 - or write down the numbers next to the symtpoms you have:
1. Hard to get up in the morning despite long sleeping
2. Difficulty remembering &/or brain fog
3. Shortness of breath
4. Palpitations
5. Light headed or seeing stars when getting up from kneeling down
6. Irritability
7. Hard to deal with stress
8. Always tired, little energy
9. Worrying
10. Negative thoughts
11. Tired or exercise intolerance
12. Chest pain
13.IBS (gas, bloating, going to bathroom)
14. Depression
15. Tightness in chest throat or stomachs
