A long time ago I read and still have the book, The Feeling Good Handbook by David D. Burns, M.D. It is for anxiety and depression. I went through it a long time ago (the copy I have was copywritten in 1999) so I can't really remember it. I still see it in book stores so it must be still requested. Now that you have reminded me of it maybe I need to go through it again.
