We have a 12 year old daughter that we cant get to school or the doctor. We have tried everything but she can't seem to get the courage to leave the house. The only doctor that will come to the house wants cash and that's not an option for us. how do we get her out of the house for treatment.

Your child has a genuine fear of something or somebody. The comment by Gloria_carr is a good suggestion, and she is a mother and grandmother.
Could you give us so more details so maybe someone in this venue really may have information or idea that will help you?
Did your child suddenly not leave the house at all, or only to school, or the doctor?
Does she go out in the yard or anywhere with you?
Is it possible she was bullied by someone at school? That has become a big problem in some countries.
Will the school counselor consider coming to your home to talk with your daughter alone, as your child might open up to her if there is a school issue at the bottom of this?
You probably tried everything you can think of, so please provide more information as someone in this venue may have experience with your situation and can help.

Since she won't go to the doctor now, did a doctor actually see her at some time and diagnosed her with agoraphobia? How long ago was it when she wouldn't go to school? nstllworth offered a very good suggestion if it is agoraphobia. Has she said anything to you why she stopped going to school?
Now in a different direction, I know when I first experienced the anxiety part of my PTSD, I would spend days inside my home. Still have difficulty in grocery stores, can only handle about 30 minutes getting exactly what I needed and out the door.
How would she respond to home schooling during this situation? It would take her attention away for a while from what is scaring her. Still believe the school counselor may be of help and may be able to come to your home.
Have you tried talking thru her phobia (beginning to end) to help her process the fear and feel comfortable?

I am sorry I can't help from a lived experience - I did ask around for some ideas because I really didn't know where to start.
Here's a tip my friend shared with me
Her daughter's psychologist asked her daughter to write down all of her fears in little notebook. Every time she felt afraid, she was to write down where she was, the time of day, the thoughts that went through her mind, and her feelings in her body. Then she was given an exercise that was very effective. For fifteen minutes each day, she had to sit in her room and conjure up her fears. She was to set the kitchen timer, to make sure that she felt the fear for exactly fifteen minutes. When the timer rang, she could stop being afraid and go about her life.

My insurance offers an option to see a doctor online thru webcam. Maybe you can research some sites that would be affordable and reputable. Did you try calling the doctor and explaining her situation? I'm sure she's not the only person that fears leaving the house and the doc maybe able to phone in a prescription. Ask her pediatrician what options are available to your daughter. I pray it all works out for her and she soon finds relief. I know it's scary. Good Luck
You got to bring her ..I know easier said than done..will a therapist talk to her on phone? Or talk therapy.com I think she can do online...I would just tell her get in car just the way you got her to Dr for shots...I don't understand if she has agoraphobia or if she's afraid of drs ...has she been bullied at school? Does she have a best friend that you can enlist to help? Maybe the friend could go with you guys? I'm so sorry it is such a difficult age. Maybe her pediatrician can prescribe something to calm her down just so you can get her into car and to appointment. Kids don't know that most people are just concerned with themselves...I know how crippling being self conscious can be. Is there a counselor at school that could talk to her? My thoughts and prayers are with you guys.
It makes me sad to hear how younger people especially ,cannot bring themselves to go to school and other activities;this experience happened to me when i was around 16 and half years;my life changed drastically overnight--where I went to work quit happily,then my father foced me to work with him and basically never really understood at the time how miserable and self concious I became --and now -im an awful lot older-feel I wasted my life and still do--anyone relate PM me🐼🐶🐵😊🐿️
maybe you could try to distract her mind on the way to the doctor. Try using a blindfold or sunglasses etc..and headphones with music. Something to block out the sound Also as an option you can give her something to relax her before you leave, like benedryl or melatonin chews for kids or whatever you are comfortable with. She could try the 'Headspace' phone app it has meditations for children and adults for specifically dealing with anxiety issues its really good helped me with my panic attacks.
