It sounds as though you've been given a diagnosis and no sense of direction, which is very unfair to you. Type 2 diabetes is a disorder of carbohydrate metabolism so that's what you need to tackle, your carbohydrate intake. If you keep that at the forefront, rather than weight loss, you could be among the many who manage to reverse your diabetes. The 12 week plan will not be the best way for you.
I'm going to give you a lot of things to read. I don't want to overwhelm you so just take your time working through them.

I'm in the same boat as you and I joined here recently because I was desperate for help. I was diagnosed during lockdown and then left totally in the dark by the professionals. The advice and support I have received here has been invaluable and extremely informative.
I am spending time reading all the links sent to me and learning to navigate this site but just knowing there are others in a similar position or with knowledge to share has already been a big help.
Dealing with Diabetes T2 and eating is a definite learning curve for me as I'm having to retrain my brain and change old (bad) habits but I'm sure I'll get there.
