Hi everyone,I'm 8 weeks pregnant.

Please let me know what you think of my results.

Took test fasted, leaving a 24 hour gap between Levo and 12 hour gap between lio and the test (split my dose the day before my test. I'm on 162mcg levo and 10mcg lio.

FT4 21.7 (10.8 - 25.5) 74.10%

FT3 5.8 ( 3.1 - 6.8) 73%

TPO 193 (0-34)

TSH- not tested by lab even though requested.

My test results from 4 weeks ago when I was on 125mcg levo and 10mcg lio.

Free T4 19.5 10.8 - 25.5 (59.2%)

Free T3 6.1 3.1 - 6.8 (81.1%)

TSH 0.002 (0.27-4.2)

So, the consultant who managing my pregnancy did an unexpected tsh and T4 test at 2pm (so about 12 hours after my dose of levo and lio). It showed that I was over range and suppressed tsh. I explained about the needing to follow the correct protocol and his argument was that because of the way t4/t3 varies over 24 hours Is that us why he goes by the TSH 🤦🏾‍♀️. Is there any likelihood that my T4 is over range or too high? I am wondering if I should have left my lio and levo dose the same as my t3 has deteriorated a bit although my T4 has gone up.

There are others on this board - you might want to use the search function. And I know others will weigh in here.

I’ll just say that mid to upper range Free T4 is what you want in the first 20 weeks. See link above for articles on general guidance for Levo increases and caveats too.

At 20 weeks your baby’s own thyroid function kicks in. Until then the baby relies entirely on your thyroid hormones.

Thank you for the reassurance and the link to the other post. My Consultant is saying similar things about strongly recommending coming off the lio for two weeks and then retesting my TSH. But he also acknowledged that I may have received different advice.

Many congratulations. I hope it all goes well.

Is the consultant a NHS or private? Your frees look good. They do want to see TSH in trimester specific ranges during pregnancy.

Thank you🙏🏽. Yes it's an NHS Consultant. I am om immunosuppressive medication this pregnancy so I'm under the care of a specialist team. Yes, he said he wanted to see my TSH between 1 and 4 but it will most probably stay low on suppressed on my relatively low dose of T3.

Good luck. I am surprised the endocrinologist is not insisting you come of the T3. Such a mix bag of experiences.

Oh yes I forgot to say it was his suggestion that I stop the T3 and stay on my dose of 162.5mcg levo! Thank you

TSH has a circadian rhythm and varies over 24 hours. So he's being dishonest in explaining his reasons for what he believes.

Free T3 follows a circadian rhythm too, just a few hours behind TSH, but fluctuates far less than TSH.

Free T4 doesn't have an obvious circadian rhythm.

Thank you for this. He's either being dishonest or doesn't know. It seems that my T4 varies quite a lot over 24 hours. The T4 results from the suprise test they did which was about 12 hours after last dose of levo and shorter gap for lio ( don't remember) was 27.5 (11.9 - 21.6) showed me as 160%! Whereas the test I did 5 days later following the usual testing protocol had my T4 at 74.1%! Do you think that there is any possibility that I'm over treated based on these widely different results?
