New Thyroid Results - Conversion Issue?

Hi,

After an emotional Thyroid rollercoaster these past couple years (post partial thyroidectomy 12/2/21 in combination with an Endo who doesn't believe in compounded medication), I finally seem to be settling into a medication and dose that works okay. I'd have to say that I'm still a bit tired and more anxious than my low stress life indicates would be normal. Not sure what to do now. I am taking 88 mcg Tirosint/day (75 mcg was definately too low and put me into a dysphoric state - and 100 mcg was too high and caused me to be irritable and anxious).

My current results look pretty good to me, but I'm wondering if I have a conversion issue. I have been inconsistent in taking vitamins because they tend to upset my stomach - I don't do well with Vit D and the B vitamins. I used to take Selenium every day until my endo told me to stop. I'm ready to start again. But I want to know what is the most important suppliment to take to help with conversion and how much.

I know myself, and one or two suppliments to start work best. I know selenium does not upset my stomach, so I can add one more to that one, and see how I do. Too many and I get overwhelmed. I so appreciate your guidance. Warmly, Lisa

TSH .032 [range .4 - 4.5] - according to "Biochemical Marers Reflecting Thyroid Function" when on T4 monotherapy after thyroidectomy one is closest to euthyroid at a TSH level of .03-.3

Free T3 2.9 (37.5%) [range 2 - 4.4] - the highest I've ever been was 3.01 when I added 5 mcg Cytomel - which made me really jittery

Free T4 1.33 (53%) [range .8 - 1.8]

Reverse T3 18 [range 9.2-24]

If you are getting jittery increasing your dose (which you do need to do) then you likely have low vitamin levels and possibly need to try a different brand of Levo.

When you do a thyroid test do you leave 24 hours between last dose of Levo & the test?

On Levo only one would expect your FT4 to be much further up the range at the 90% level so you are quite a bit under replaced currrently.

Your FT3 is also too low and should be at 50%+, even 60-70%.

Suggest you get vitamins tested and make a new post with the results.

WhichB vitamins have you tried and could it be the excipients you have an issue with?

Hello there I had a total thyroidectomy in 2022 due to graves disease.I was OK at first on T4 only(levothyroxine) but started to experience conversion issues last year.Over here in the UK it is difficult to get T3 prescribed alongside levothyroxine so I am trying to improve conversion myself.I have a poor appetite and tend to eat the same foods every day.I also seem to be having problems processing sugar and carbs(I have been tested and am not diabetic)I got symptoms of excess urination at night and just a funny feeling.I stopped food after about 7pm but have since learned that conversion needs carbs so that may have caused my problem.I now eat regularly and make sure I have enough calories during the day.

The vitamins I take are vitamin D3+K2 as I also found my Vitamin D levels had gone back down after supplementing before.I take a high strength vitamin C.I have taken that since I was on carbimazole for my graves and continue to do so.I take magnesium glycinate but only 1 a day as anymore upsets my stomach. I take a combined zinc and selenium(I have thyroid eye disease).I have just started a super B complex but only take 1 tablet a day.I have also just started omega 3,6,9 but again only 1 a day to start.

I am definitely feeling better with the vitamin D3.My levels had gone down to 60something and they should be over 100.I have more energy.

I started each supplement separately and gave a couple of weeks for it to settle before introducing a new one.Also if it recommends say 3 tablets a day I start with 1 to see how I go on.I would think something is better than nothing if I can't tolerate the full dose.

I did need a dose increase of my levothyroxine and try to split it evenly across the week by cutting tablets.My endo said 125mcg 3x a week and 100mcg 4x a week so I split the 25mcg and take 112.5mcg 6x a week and 100mcg 1x per week so my dose is more even.I do feel better.

I am going to leave it till about Easter and test everything again to see where I am.I have been doing private tests myself as it's difficult to get doctors to test everything I need.

Thank you so much. Even though I live in sunny California my vitamin D levels are on the low end of the range. Last tested over a year ago was 60. I will start with a small amout of D then move to one of the Bs. I really appreciate the encouragement.
