Hello.I have just received my latest blood test results and was hoping for some feedback before I see my doctor next week to review my Levothyroxine dosage. I should mention that since I last posted I have been diagnosed with Lichen Sclerosis ( another autoimmune disease )
CRP 5.570 ( range < 3 )
Ferritin 280 ( range 30-264 )
Folate 36 ( range > 7 )
Vitamin D 116 ( range 50-250 )
TSH 0.750 ( range 0.27-4.2 ) - Up from 0.412 last Oct 2023
T3 4.2 ( range 3.1-6.8 )
Thyroxine 18.9 ( range 12-22 )
Thyroglobulin antibodies 391 ( range 0-115 )
Thyroid Peroxidase 149 ( range 0-34 ) Up from 104 last Oct.
Should I be changing my dosage or staying at 75 for the time being.

Reading your helpful profile you are 100% gluten free, almost dairy free
Might help cutting out dairy completely
No B12 result
Other vitamins are good
Was test early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test
FT4: 18.9 pmol/l (Range 12 - 22)
Ft4 69.00% through range
FT3: 4.2 pmol/l (Range 3.1 - 6.8)
Ft3 only 29.73% through range
Worth trying small increase in Levo before considering getting T3 prescribed
Approx how much do you weigh in kilo?
Guidelines on dose by weight approx 1.6mcg per kilo of your weight per day
If GP won’t increase or if increase doesn’t increase Ft3

Ideally choose an endocrinologist to see privately initially and who also does NHS consultations
I currently weigh 70kg. I have tried cutting out dairy altogether but it didn't make any difference to symptoms.
The blood test was done around 8am, only water and at least 24 hours after the previous dose of Levo ☺️
Please forgive me if these are stupid questions but would a delight increase in Levo bring the antibody numbers down ? Also why would I need to be prescribed T3, will that be in addition to the Levo ?
Just need to get my facts straight before I speak to my doctor.
Ideally your Ft4 (levothyroxine/storage hormone ) should be at similar percentage as Ft3 (active hormone)
Naturally in a correctly working thyroid it would make 80-90% Ft4 but (crucially ) would also make 10-20% Ft3
Standard treatment is to just offer Ft4 (levothyroxine) as replacement thyroid hormone
BUT at least 20% of thyroid patients, especially if autoimmune and/or after menopause find their conversion of Ft4 to Ft3 is very poor……results in high Ft4 and low Ft3
Many many thyroid patients benefit from the addition of SMALL doses of T3 prescribed ALONGSIDE Levothyroxine
Usually as 2 or 3 x 5mcg per day
Many members on levothyroxine plus small dose T3 find they do best when BOTH Ft4 and Ft3 around 60-70% through range
because of recent exploitation and drastic increase in price of T3 …..up 6000% in 2016…..NHS became reluctant to prescribe
Price now dropped significantly and getting easier to get prescribed……but waiting lists of over a year means private is best way to get trial initially
