my latest results , still feeling poorly what do they mean ?

Serum TSH level 0.72 mu/L [0.27 - 4.2]

Serum free T4 level 16.4 pmol/L [12.0 - 22.0]Blood haematinic levels

Serum vitamin B12 level 301 ng/L [197.0 - 771.0]

Serum folate level 3.6 ug/L [3.0 - 20.0]

Serum ferritin level 237 ug/L [13.0 - 150.0]; Raised Ferritin - ? liver problem ? reactive.Serum total 25-hydroxy vitamin D level 42.0 nmol/L; NB:Reference comments altered to advise treatment.

<25 nmol/L : Vit D deficiency,requiring replacement

25-50 nmol/L : Vit D insufficiency,consider replacement

50-150 nmol/L : Adequate Vitamin D status.

There's no FT3 result so we can't see how you are converting. You likely need to get that done privately.

Free T4 (fT4) 16.4 pmol/L (12 - 22) 44.0%

Your FT4 is too low. I would expect that to be nearer the top of the range for you to feel better. Insist on a dose increase, perhaps ask for it as a trial which docotors find easier to accept and agree to.

Your vitamin levels are terrible. With high ferritin ask GP for a full iron panel.

Ask GP to test for coeliac.

Are you vegan or vegetarian? If not...

B12 - do you have symptoms of B12 deficiency? The reference range for B12 is very wide and cut off point too low. theb12society.com/signs-and...

If you do then you should discuss this with your doctor for further tests for Pernicious Anaemia.

If not, then start with a methyl B12 sublingual spray or lozenge for a week, then add a good B complex. Once you run out of the separate B12 just continue with the B complex.

Folate - aim for a level of 20. Recommend taking an active/methyl (should say on label) B complex which contains 400mcgs folate (also B12). Slightly cheaper options with inactive B6:

What supplements are you taking?

Vitamin D should be around 100 - 150. Buy one that includes vit K2 to help it go to your bones. Some are available in oil or you can take it with an oily meal for better absorption. are you currently taking any vitamin D

GP should prescribe 1600iu everyday for 6 months

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and a week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

Have you had thyroid antibodies tested yet

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

I didn’t take Levothyroxine on the moring of my test , on my results it says borderline or satisfactory, I’m seeing a thyroid doctor on Monday so hopefully he will be able to advise me on what to do , my doctor did request an antibody test , but not on my results , I’m realy suffering with my throat feels like something pushing on a nerve , and I can’t walk properly sometimes awful feeling , does this sound like hashimotos

Ft4 44.00% through range

So plenty of room for next dose increase

Many (most?) people on just levothyroxine need Ft4 (levothyroxine dose) at least 60-70% through range

Vitamin levels

GP should advise or prescribe vitamin D

Very unlikely to ever comment on B12 and folate as they are within range

Down to you to self supplement to improve
